| Objective:To find the similarities and differences of the treatment of knee osteoarthritis by Kidney-Supplementing Blood-Quickening Chinese Medicine and Celecoxib Capsules by experimental and Clinical research for further clinical promotion and utilization to treat knee osteoarthritis by Kidney-Supplementing Blood-Quickening Chinese Medicine or Celecoxib Capsules.Methods:Experimental research:24 News Zealand rabbits were randomly divided into 4 groups,marked A,B,C and D.the rabbits of group A,the normal group,were not injected papain,only fed with fodder 7 weeks,the rabbits of group B,C and D were caused to suffer from osteoarthritis by being injected papain into their left knee joints two times.The method of inducing OA model was used first by iitoh Y Katsuvamaki T(Japanese researchers).One week later,the models had been made successfully,the rabbits of Group C began to be treated with Kidney-Supplementing Blood-Quickening Chinese Medicine for 6 weeks; Group D treated with Celecoxib Capsules for 6 weeks;Group B,blank contrast groups,were treated with nothing and fed with only fodder.6 weeks later, all the rabbits were killed and The gross morphological condition was observed and grade of cartilage surface was evaluated with megaloscope according to arthroscope grade method.Specimens were taken from the weight-bearing portion of the femoral condylar and were examined histologically and graded with C. Colombo criteria.Clinical research:50 outpatients who were diagnosed knee osteoarthritis were divided into Kidney-Supplementing Blood-Quickening Chinese Medicine group and Celecoxib group randomly,which would be treated with Kidney-Supplementing Blood-Quickening Chinese Medicine and Celecoxib Capsules for 4 weeks separately.Observation indicatrix was established according to the revision classified standard of American rheumatism association's osteoarthritis in 1995 and TheⅡissueⅢof clinical test plan for knee osteoarthritis.When the treatments were accomplished,the changes of main symptoms and signs were observed,the contrasts were performed between the same group and the different groups.Result:Experimental research:1.Roughly observed result:Pathology demonstrated roughly that the articular cartilage of group A was normal,the articular cartilage of B-D groups had draw back changes of varied degree.2.The result of the pathology grade:the pathology grade of group B is higher than that of group A,which had statistical significance(P＜0.05),the pathology grade of the treatment groups was lower than that of group B(P＜0.05),the pathology grades of group C was better than the pathology grades of groups D,which indicated that all the treatment groups could improve the protein polysaccharide contents of the cartilage matrix,and the group of C had the better result.Clinical research:1.Comparison of integral:Compared with the integrals of symptoms and signs before treated,all the integrals in Kidney-Supplementing Blood-Quickening Chinese Medicine group and Celecoxib group had significant difference after the treatment course had ended,which showed that both Kidney-Supplementing Blood-Quickening Chinese Medicine and Celecoxib could relieve the symptoms and signs related to knee osteoarthritis;2,Comparison of integral change:The integral change of patient's symptoms and signs between Kidney-Supplementing Blood-Quickening Chinese Medicine group and Celecoxib group did not have significant difference(P＞0.05),which showed that the integral change between Kidney-Supplementing Blood-Quickening Chinese Medicine group and Celecoxib group did not have statistical significance, but the muscle power integral(to count time of raising limb straightly), the function integral change,the ache integral and score of ache rule change had significant difference(P＜0.05),which indicated that Celecoxib was better to alleviate the osteoarthritis patient's ache symptom,but Kidney-Supplementing Blood-Quickening Chinese Medicine was better to improve the patient's function;3,Comparison of clinical curative effect:The total clinical curative effect did not have significant difference between Kidney-Supplementing Blood-Quickening Chinese Medicine group and Celecoxib group(P＞0.05),which indicated that both Kidney-Supplementing Blood-Quickening Chinese Medicine group and the Celecoxib group had the same result to relieve the clinical symptoms and signs.Conclusion:1.The method of injecting papain into the knee joint of rabit can successfully induce the model of OA.2.Both Kidney-Supplementing Blood-Quickening Chinese Medicine and Celecoxib could promote the cartilage matrix to synthesis,suppress the cartilage matrix to decompose,then to promote the damage articular cartilage to repair,.but Kidney-Supplementing Blood-Quickening Chinese Medicine is better.3.The clinical research showed that the total clinical curative effect between Kidney-Supplementing Blood-Quickening Chinese Medicine group and Celecoxib group did not have significant difference,but Kidney-Supplementing Blood-Quickening Chinese Medicine had certain superiority to improve the function of knee and increase the power of muscle.But for releasing pain, Celecoxib is better than Kidney-Supplementing Blood-Quickening Chinese Medicine.doctor can use Kidney-Supplementing Blood-Quickening Chinese Medicine or Celecoxib to treat OA by according the difference of patient' s symptom or signs.
