| ObjectiveIt is high recurrence following surgery in lacrimal epithelial tumors. We have analyzed the clinical features of recurrent lacrimal gland tumors in order to understand the tumors more deeply. On the other hand, apoptosis has very close relationship with genesis and development of tumors. Survivin is a new inhibitor of apoptosis protein and caspase proteases are keys of apoptosis, especially caspase-3. Survivin and caspase-3 are hot spot of oncogenesis, but there is no correlated report in lacrimal gland epithelial tumors. The study investigated the expressions of survivin and caspase-3 in lacrimal gland epithelial tumors and analyzed the relationship and clinical significance of two factors and lacrimal gland epithelial tumors.Methods1.Collect the patients of lacrimal gland epithelial tumors from January 2003 to November 2007 in Tianjin Eye Hospital and retrospective analyze. To summarize and analyze the clinical and imaging features of recurrent lacrimal gland epithelial tumors.2.Collect specimens of lacrimal gland epithelial tumors in Tianjin Eye Hospital from January 2003 to November 2007 and normal lacrimal gland.3.Stained with survivin and caspase-3 with immunohistochemistry.4.Statistical analysized by SPSS 10.0.Results1.36 cases of lacrimal gland epithelial tumors were all collected. 18 cases of pleomorphic adenomas, 6 cases of adenoid cystic carcinomas, 1 case of adenocarcinoma and 11 cases of recurrent lacrimal gland epithelial tumors. There are 5 cases of pleomorphic adenomas, 5 cases of adenoid cystic carcinomas, 1 case of pleomorphic adenoma canceration in recurrent lacrimal gland epithelial tumors.2.The recurrent lacrimal gland epithelial tumors showed orbital destruction and irregular boundary. There are no significant differences in sex, eye and age among pleomorphic adenomas, adenoid cystic carcinomas, recurrent pleomorphic adenomas and adenoid cystic carcinomas. In orbital destruction and irregular boundary, there is significant difference between recurrent pleomorphic adenomas and pleomorphic adenomas, but no significant difference between recurrent pleomorphic adenomas and adenoid cystic carcinomas.3. 33 cases of lacrimal gland epithelial tumors and 8 normal lacrimal gland were all collected. 17 cases of pleomorphic adenomas, 6 cases of adenoid cystic carcinomas, 5 cases of recurrent pleomorphic adenomas and 5 cases of recurrent adenoid cystic carcinomas.4. The expression of surviving increased with malignancy. There are significant differences between recurrent and initial pleomorphic adenomas and normal lacrimal gland in the expression of survivin while there is no significant difference between those and adenoid cystic carcinomas. No significant difference in the four types of tumors in expression of caspase-3. Survivin and caspase-3 are negative correlation.Conclusions1. Both benign and malignant recurrent lacrimal gland epithelial tumors are malignant invasion.2. Although the recurrent lacrimal gland pleomorphic adenomas are benign in pathology, they are malignant in biological behavior.3. The expression of survivin is correlated to malignant degree.4. The recurrent lacrimal gland pleomorphic adenomas are malignant in molecular biology.5. In lacrimal gland epithelial tumors, survivin inhibits caspase-3 so that inhibits apoptosis, and then cells malignant hyperplasia and induces recurrence of tumors. |
