The Clinical Study Of Treatment For Cervical Spondylosis Of Vertebral Artery (CSA) By Needle-knife Therapy

Research purpose:To analyses the scarcity of the literature about needle-knife therapy for CSA;to analyses the clinical result and security of needle-knife therapy for the patients of CSA by evaluating the clinical symptoms and signs before and after treatment and changes from daily life, work,psychological and society according to clinical test and questionnaire in order to provide secure and effective method for the patients for reducing the clinical symptoms and improving the quality of life;to summary the clinical experience about needle-knife manipulation and analyses the reasons between anesthesia and needle-knife for providing the basis of improving the clinical treatment and the manipulation.Research method:Analyses the literature about needle-knife therapy for CSA during the recent 20 years.Observe the clinical efficacy of needle-knife therapy for CSA by evaluating the clinical symptoms and signs and changes from daily life,work,psychological.Method:The 180 cases were divided into two groups as acupuncture group with 90 cases and needle-knife group with 90 cases and use acupuncture and needle-knife to cure the corresponding group and follow-up for one month respectively.Then select 30 cases with anesthesia and 30 cases without anesthesia from needle-knife group randomly to study.Research result:1,The clinical efficacy and security study of needle-knife therapy for CSA. The research divide 180 cases into two groups as acupuncture group with 90 cases and needle-knife group with 90 cases randomly.There were no difference between two groups in gender,age,somatotype,course of disease, syndrome differentiation,vital signs and state of illness before treatment.According to the clinical symptoms and signs and changes from daily life and psychological of patients,the needle-knife therapy has significant clinical result for CSA,the total efficient reach 91.11%.The therapy can reduce some clinical symptoms effectively such as dizzy,headache,nausea, vomiting and tinnitus,can improve the positive signs and adaptation capacity of sociality and psychological.During one month,the needle-knife therapy for CSA has better clinical efficacy than acupuncture therapy.The effect duration is longer than that of acupuncture therapy.The needle-knife therapy has better function than acupuncture therapy in reducing the degree,frequency and duration of dizzy.The needling sensation of needle-knife therapy is stronger than that of acupuncture therapy.The needle-knife therapy can acquire better clinical efficacy due to duration.Among the 90 cases from needle-knife group,there were no significant difference about vital signs between before and after treatment,and there were no occurrence of nerve or vascular injury or other harmful matters.2,The relationship between anesthesia and needle-knife therapy.Among the needle-knife group,the 35 cases with anesthesia account for 38.89%,the 55 cases without anesthesia account for 61.11%.Anesthesia can reduce the pain feeling but decrease the duration of needling sensation.Anesthesia or not has no effect on the clinical efficacy of needle-knife for c CSA.3,The study of relationship between the CSA and the patients' age,gender, size,course Of disease,syndrome differentiation.Among the 180 cases of CSA,the amount of female is more than male;the age stage is mainly between 50 to 65;the incidence is related with body size, there is to say,the fat or overweight patients is sick easily.Among the 180 cases of CSA,syndrome differentiation is the deficiency of qi and blood,which is related with that the disease occurs in the old people easily. 4,The analyses for reasons about clinical efficacy of needle-knife.Among the 90 cases from needle-knife group,the clinical efficacy of needle-knife is not related with age,gender,somatotype,course of disease, syndrome differentiation.5,The literature study of needle-knife therapy for CSA during the recent 20 years.From the literature about needle-knife therapy for CSA during the recent 20 years:5.1 The shortage of the clinical research:the homogeneity and the heterogeneity of research objective are scarcity;there is defect in design of research method;the treatment method is indefinite and complicated;there is no unified standard in the clinical efficacy;the statistics method is not clear;which decrease the effectiveness,authenticity,reliability and rigor degree of literature.5.2 The highest clinical efficient of needle-knife therapy is 100%for CSA,but the lowest is 81.33%,which indicates that needle-knife therapy is one of effective method.However,the clinical credibility decreased because many literatures combine with other methods.5.3 The literatures about influence of needle-knife therapy on auxiliary examinations are little.The recorded auxiliary examinations include TCD, SPECT,blood rheology,hair trace element detection and nailfold microcirculation change,but these auxiliary examinations cannot be the clinical efficacy standard of needle-knife therapy for CSA due to the lack of extensive and comparability.5.4 The is no occurrence of harmful matters in the literatures,which indicates that the needle-knife therapy is a kind of relative secure treatments and is worth spreading and applying.Conclusion:1,The needle-knife therapy has the significant result for the patients of CSA,the clinical efficacy of which is better than acupuncture therapy;the total efficient reach 91.11%.2,The needle-knife therapy has the stronger stimulation,the longer duration and the better function.3,The needle-knife therapy is not restricted with age,gender,size,course of disease and syndrome differentiation,is worth spreading and applying in clinic.4,The anesthesia is a kind of auxiliary method during the needle-knife therapy. In clinic,the application of anesthesia depends on metal state and psychological quality,which make the patients receive the comfortable and effective treatment.5,The needle-knife therapy for CSA is a kind of relative secure treatments and is worth spreading and applying.6,From the literatures,there are some shortages of the clinical research quality;the clinical effectiveness of needle-knife therapy for CSA is worth discussing;there is no occurrence of harmful matters.
