OBJECTIVE:To construct the hemodynamic models of complex intracranial aneurysms based on Computational Fluid Dynamics technique.Different border papameters were given to the model and the hemodynamic characteristics were compared, thus provide some beneficial informations to clinical treatment choice. METHODS:The three-dimentional digital models of intracavernous aneurysm, paraclinoid aneurysm, and basilar artery aneurysms were constructed using MIMICS,ANSYS and CFX softwares.Different hemodynamic modalities and border conditions were assigned to the models, related mechanical parameters were analyzed and hemodynamic status was compared between different modalities.Twenty-eight complex intracranial aneurysms, according to the information from different digital models, were treated either by simple proxmal ligation, trapping with extra-cranial intra-cranial bypass,or simple stenting methods.RESULTS:The accurate models were established successfully.Under differnent border conditions, there existed difference in blood flow volecity, wall total pressures and wall shear stress.Of the 28 complex aneurysms,4 undervent simple proximal ligations,20 accepted trapping with extra-cranial intra-cranial bypass, and the remained 4 had a simple stenting treatment. At discharge, the GOS(Glasgow Outcome Scale)were 5-4 in 24(85.7%)cases,3 in 3(10.7%)cases, and 1 in 1(3.6%)case.CONCLUSIONS:The digital models of the complex intracranial aneurysms are feasible and reliable. These models could provide some important informations to clinical treatment options.By changing the hemodynamic status, some unclippable complex intracranial aneurysms could be treated effectively.
