| IgA nephropathy is the most common primary glomerular disease and its pathogenesis is not clear at present.There is no specific therapeutic measure in modern medicine,however Traditional Chinese Medicine(TCM) treatment of this disease has certain advantages.In this study,through a retrospective analysis of clinical data to explore the regularity of TCM syndrome distribution and evolution of IgA nephropathy.And using association rules to sum up the thinking and clinical experience of my tutor treating IgA nephropathy.On the other hand,through a systematic review of clinical randomized controlled trials(RCT) of TCM treatment of IgA nephropathy to evaluate the curative effect and then to provide TCM treatment of IgA based on evidence.The innovations of my paper are next two aspects:â‘ The underlying rules of priscribing is mined from a abundant and fuzzy database through association rules analysis;â‘¡Systematic review on clinical randomized controlled trials(RCT) of TCM treatment of IgA nephropathy is done by using Revman5 Meta-analysis software provided by Cochrane Collaboration.Theoretical StudyWe systematically summarize the name of diseases,pathogenesis,syndrome distribution and treatment of IgA nephropathy through the collation and analysis of the literature.And we put the main emphasis on the pathogenesis and evidence-based treatment of IgA nephropathy in recent years through the study on Western medicine literature.Study of Characteristics and SyndromeThe research results on syndrome differentiation of 251 patients with IgA nephropathy show that the most common clinical manifestations are hematuria and proteinuria.The most common symptoms are fatigue and lack of strength,weakness of loins and knees,feverish sensation over palms and soles,or aversion to cold,etc. Red tongue,thin white coating and thready and rapid pulse are the most common tongue appearance and pulse.Distribution of the syndrome:Syndrome of Dual Deficiency of Qi and Yin,Syndrome of Liver-Kidney Yin Deficiency,Syndrome of Spleen-Kidney Qi Deficiency are common syndrome,and the proportion of three up to 92%;one of the most common syndrome is Syndrome of Dual Deficiency of Qi and Yin,accounting for 70%;up to 65.3%of the cases have accompanied syndrome.All of these state that IgA nephropathy is a disease of deficient root cause and excessive symptoms,deficiency-excess in complexity.And dual deficiency of Qi and Yin is the very central pathogenesis of IgA nephropathy.Treatment Experience of Tutor based on association rules analysisThrough association rules analysis,we find my tutor treating IgA nephropathy has the following characteristics:â‘ Syndrome differentiation and treatment in different phase:According to TCM purpose of "symptomatic treatment in acute condition,radical treatment in chronic condition",the main treatment in acute attack phase is to eliminate pathogen while in chronic deferral phase to strenthen vital qi.â‘¡Apply tonification with reduction excess as well as strengthen healthy qi and eliminate pathogen.â‘¢To select formula through grasping the main symptoms.â‘£Treatment based on the combination of syndrome differentiation,formula and syndrome correspondence,symptom differentiation.Systematic review of RCTMeta-analysis results show that TCM treatment on IgA nephropathy have a better effect in the respects of clinical efficacy,reducing moderate or small amount of proteinuria,stabling renal function.It indicates that TCM treatment on IgA nephropathy is a prospective choice and worth while to study in depth.Effect on red blood cells in urine,due to differences in measurement units causing included study inadequate and resulting in uncertain efficacy,so future studies need to be done to confirm it.Bucause of lower overall quality of included study,the efficacy and safety of TCM treatment on IgA nephropathy still need to be further confirmed by well-structured,large sample randomized,double-blind,placebo-controlled trial.
