Studies On The Primary Principles And Syndrome Differentiation-treatment Of Menstrual Disorder Treatment Based On The Liver In TCM

Objective:Starting with the literature review, studies aimed to elucidate the role of Liver in menstrual regulation of TCM based on the liver pathophysiology and menstruation cycle, to reinforce the theoretical connotation and expand the clinical application through excavating the related topic in classical works, to analysis the etiology and pathogenesis of menstrual disorder and liver-related disease, dialectical style and dialectical point, the therapeutic regimen and medicinal expertise, to refine the syndrome differentiation-treatment principle from menstrual disorder cases of the famous ancient gynecologists.Methods:1. Literature review Combined the ancient and modern traditional Chinese medicine literature, retrieved the commentary and clinical records in ancient relevant literature, studies fully revealed the regulation of traditional Chinese medicine on menstrual generation and the recognition in etiology, pathogenesis of menstrual disorder, reflected the role of liver in menstrual regulation and disorder, established the rationale of menstrual disorder treatment based on the Liver theory in TCM.2. Theoretical connotation Clarification In view of uncertain concept and relatively vague connotation in traditional Chinese medicine system, the concept were selected in the literature collection and reorganization, and given the thorough accurate explanation which could deepen the main academic argument and essential concept.3. Clinical data analysis by tabulation The clinical data were adopted the evidence collection by tabulation, gathered cases, then re-categorized and statistized them by the number or frequency according to the data entry. The relevance of internal organ, age and liver syndrome in the menstrual disorder were discussed.4. Multi-level dialectical style studies Many kinds of Chinese medicine dialectical style connotation were analyzed according to the menstrual disorder characteristic, the physiology of Visceras, qi and blood to elaborate pathology, cold and heat from the actual situation to strengthen the nature of the characteristics syndrome, which identified in the blood and qi emmeniopathy the adaptation of the dialectical style. The etiology, involved body parts and disease-dialectical three elements were seized through the whole process of the treatment principles, selected formula and then sought the effective syndrome differentiation-treatment methods.Results:From the point view of liver's physiological characteristic and the menstruation pathophysiology, the liver function was showed in the formation and regulation in the menstruation. Liver's pathology is the important pathogenic factor of menstrual disorder. As the representative of the classic masterpieces such as the Yellow Emperor's Canon of Internal Medicine, Treatise on Exogenous Febrile Diseases and Synopsis of the Golden Chamber, the clinical Significances and basic methods have been revealed in treating the menstrual disorder with the theory of liver. With the data collection and analysis of 869 menstrual disorder cases, there were more cases belong to the liver than which belong to the other internal organs by the viscera differentiation treatment.The age relevance researches of menstrual disorder revealed that there was significant difference between the liver and the kidney, while no statistical difference between the liver and the spleen in age structure ratio. In the cases of liver, the statistical results proved that there were more sthenia-syndromes than asthenia-syndrome, and heat-syndrome than cold-syndrome, put forward the differential theory which set the viscera as outlines, the eight principal syndromes as focuses then internal qi and blood. The differentiation treatments of menstrual disorder with liver symptoms were pointed out based on the complicated relationships at different levels. Named by syndrome, the 16 classic prescriptions of menstrual disorder with liver symptoms were elaborated.The classic elaborations and attainments of ancient gynecologists and clinical application of 16 conventional syndromes were induced, which demonstrated the clinical complexity and multiplicity in a higher level, and enriched the therapeutics content of menstrual disorder with liver symptoms in the theory and clinical utilization.Conclusion:These studies stressed on the researches of the primary principles and the syndrome differentiation-treatment, combined with the modern achievement of nerve-immunity-internal secretion system of liver, and the maturity of clinical research methods on the union of the disease and syndrome in TCM. With the theoretic deepening and clinical investigation development, this project surely has enormous space and bright prospect.
