| Objective:This study was to explore the effects of behavior therapy to relapsed schizophrenia (R-SCH) and recurrent major depression disorder (R-MDD), and to explore track of patients’ change, techiniques of therapists and mechanism of behavior therapy by investigating the process of therapy.Methods:1. Prospective intervention study design was adopted in this study. By coparing behavior therapy plus medication with medication alone to investigate effects of behavior therapy to R-SCH and R-MDDin symptoms, psychological function and social function. One hundred patients were enrolled, and were investigated by Symptom check list-90(SCL-90), General Quality of LifeInventory-74(GQLI-74), Social Support Rating Scale (SSRS) and Simplified Coping Style Questionnaire (SCSQ) at the time of before treatment, after treatment,3months after treatment and6month after treatment.2.Qualitative research methods including theme analysis, categorization analysis, assimilation analysis, task analysis and core conflict relationship analysi were adopted to investigate process of therapy. Subjects were two relapsed schizophrenia and one recurrent major depression disorder patients. After transferring the therapeutic video into words and coding them, change process of patients, techniques of therapists and mechanism of behavior therapy were closely investigated. Nvivo8.0qualitative data management and analysis software was used to assist the data analyzed process.Results:1. when at6months after treatment changed scores in somatization factor of SCL-90, objective support factor of SSRS and negative coping style of SCSQ of behavior thrapy group were siganificantly higher than control group, although there were no significant differences between behavior therapy group and control group in the changed scores of questionnaires at the end of treatment and3months after treatment.2.Effect of treatment is difference between different therapists, and the mix model shows that therapist1had more significant efficiency than therapist2.3.Behavior therapy can improve social function of R-SCH, and it can improve coping style and social support of R-MDD.ã€4.The longer of follow up time, the more obvious effect behavior therapy exihibited.5.The change processes of patients were not monothonic depend the result of assimilation analysis, relationship frame and core conflict relationship analysis.6.The most common techniques used by therapist were analysis of behavior background, exploration of behavior background, exploration of constructive behavior, analysis of behavior function, suggestion of constructive behavior, analysis of behavior model, instruction of behavioral plan, analysis of stimulation function.7.Behavior therapy was worked through change the interaction between behavior and outside stimulates by change behavior model, as while as through reconstruct the relationship frame.Conclusion:(1)It shows that behavior therapy have little effect on improvement of symptoms of R-SCH and R-MDD, but it has positive influence on psychological and social function on these patients.(2)Different therapists showed different effect, although the same psychotherapy method was applied.(3) In the process of behavior treatment, patients promoted reluctantly.(4) Therapists should adjusted their technical depending on various problem patients confronted and status of their problems.(5) When qualitative methods was used in psychotherapy, them may greatly helpful in monitoring status of patients and adjusting techniques of therapists. |
