Purpose:Through the analysis of the correlation between FRAX and its fracture risk factors and the micro-structure and nano-structure and mechanical properties of the bone trabecula, discuss the mechanism of FRAX tool in prediction of the fracture incidence of osteoporosis patients, and the risk difference of the influence of different fracture risk factors on Chinese micro-structure and nano-structure and biomechanical properties of the bone trabecula, thus to provide the data support for FRAX tool to adjust the weights of different risk factors in the fracture risk algorithm for the Chinese population.Method and Result:(Chapter I) FRAX could separate some high-risk patients from people with osteoporosis before osteoportic fracture occurs, however, its accuracy to predict the incidence of osteoportic fracture in China is low, and the relevancy between some risk factors and fracture incidence of osteoporosis patients in China is not as apparent as that of other population.Therefore, the research on the correlation between FRAX fracture risk factors and the change in bone quality of osteoporosis patients in China area is helpful to improve the accuracy of predicting the incidence of osteoporosis fracture in this area. (Chapter II) Patients with osteoporotic fracture have less trabecular bones, which become thinner and less, and the connection points between the trabecular bones lessen, which make intensity of the spongy bone decrease. The damage of micro structure of the trabecular bone and the decrease of the biomechanical property is the vital basic pathology of the fracture for the patients with osteoporosis.(Chapter III) FRAX predict that there is low correlation between the probability of major fracture and hip fracture in the caput femoris displacement patients in China and the micro structure and nanostructure and the change in mechanical property of the bone trabecula. And in the fracture dangerous factors, the age and bone density closely relate to the microstructure and nano-structure and the change in mechanical property of the bone trabecula, and there is obvious gender difference in the microstructure and nano-structure and the change in mechanical property of the bone trabecula, and smoking can destroy the micro-structure and nano-structure and decrease the mechanical strength.Conclusion:The findings from the perspective of microscopic bone structure and mechanical properties prove FRAX reasons have low prediction accuracy of China osteoporotic fractures occurred:Chinese population bone mass has its own characteristics, and the association between fracture risk factors of FRAX and microstructural and mechanical properties of trabecula bone is not the same as the white race in the West. Application of FRAX in China need to consider the characteristics of the Chinese population bone mass.
