The Clinical Study Of Using Acupuncture Based On Combination Of Yuan And Luo Points Method And Cognitive Therapy On Depression Of The Liver Qi Stagnation Syndrome

Objective To evaluate the clinical effect with combination of Yuan and Luo points acupuncture and cognitive therapy for depression of the Liver qi stagnation syndrome,and approach mechanism of combination of Yuan and Luo points acupuncture and Cognitive therapy.Method Using randomized controlled single blinded testing method,96qualified menopausal syndrome patients were divided into combination of Yuan and Luo points acupuncture and cognitive therapy group、drug group and acupuncture group.Because of the defluxion during the treatment,we actually have accomplished the treatment of89cases.The treatment group was treated with combination of Yuan and Luo points acupuncture and cognitive therapy,it has30cases; group treated with drug (Fluoxetine) and group treated with acupuncture,each group has31、28cases respectively.We take6weeks as one course of treatment,and consecutively observe one course.We filled in clinical manifestation observation charts before and after the treatment. The effects were evaluated by the HAMD Scales, the HAMA Scales,the SDS Scales, and the improvement Scales of symptoms of Traditional Chinese Medicine before thetreatment and4time points (at the end of2week,4week and6week)during the treatment course, so as to observe the amelioration condition of patients’ clinical manifestation after the treatment. Meanwhile, we tested the level changes of NE、DA and5-HT in patients’blood plasma before and after the treatment.Then we follow up patients with the HAMD Scales4weeks later when the one course of treatment being finished.At last,we evaluated the security of combination of Yuan and Luo points acupuncture and cognitive therapy referring to Adverse reaction of record. Results (1)BaselineThere were no statistically signficant in the comparability of baseline characteristics of patients in the the three groups in gender, age disease duration,HAMD points before treatment,SDS Scale,Scale score before treamtment and the traditional Chinesemedicine(TCM)symptom test before treatment, indicating that the baseline data between three groups were comparable.(2) Total effective rateThe total validity rate of combination of Yuan and Luo points acupuncture and cognitive therapy is83.33%,while the group treated with drug is77.42%、group treated with acupuncture is67.86%. There is significent difference between combination of Yuan and Luo points acupuncture and cognitive therapy group and acupuncture group(P<0.05). There isn’t significent difference between combination of Yuan and Luo points acupuncture and cognitive therapy group and drug group (P>0.05).(3) HAMD ScaleThe gross effective rate was higher and the HAMD Scale score was better in the combination of Yuan and Luo points acupuncture and cognitive therapy group than that of the drug group and acupuncture group (P<0.05).The improvement of score of anxiety/somatization factor, body weight factor, cognitive handicap factor, blockage factor and the sleeping disorder factor in combination of Yuan and Luo points acupuncture and cognitive therapy group was started on the2week during the treatment, and which was lasted to the end of the treatment.(4) SDS scaleThe SDS Score was better in the combination of Yuan and Luo points acupuncture and cognitive therapy group than that in drug group and acupuncture group(P<0.01), however no significant difference was found between the drug group and acupuncture needle group(P>0.05),yet,the effect of drug group is better than acupuncture group.(5) Improvement Scales of symptoms of Traditional Chinese MedicineThe Improvement Scales of symptoms of TCM Score was better in the combination of Yuan and Luo points acupuncture and cognitive therapy group than that in the drug group and acupuncture group(P<0.01),again, no significant difference between the drug group and acupuncture group.(6) We take SERS as the judgement scale of side-effect.The SERS of combination of Yuan and Luo points acupuncture and cognitive therapy group is better than drug group and acupuncture group after4week treatments,yet there is no significant difference between the drug group and acupuncture group at all time.(7)Follow-up scaleWe gave patients follow-up with HAMD scale4weeks later after treatment.There is significant difference among three groups.The HAMD scale of combination of Yuan and Luo points acupuncture and cognitive therapy group is better than drug group and acupuncture group,yet there is no significant difference between the drug group and acupuncture group.(8)The NE、DA、5-HT content in blood plasmaCombination of Yuan and Luo points acupuncture and cognitive therapy can improve the level of DA, NE,5-HT in patients’ blood plasma.But acupuncture cann’t improve the level of DA and NE.Drug is better than combination of Yuan and Luo points acupuncture and cognitive therapy in improving the level of5-HT in blood plasma of patients.Conclusion (1) Combination of Yuan and Luo points acupuncture and cognitive therapy、drug and acupuncture has certain effect in alleviating the degree of depression of the Liver qi stagnation syndrome.But the Combination of Yuan and Luo points acupuncture and cognitive therapy has better durable effect than other two groups.lt indicates that acupuncture based on combination of Yuan and Luo points method cognitive therapy is effective in curing depression of the Liver qi stagnation syndrome,and have good security,it is worth studying and spreading in clinic.(2) Acupuncture based on combination of Yuan and Luo points method and cognitive therapy regulating liver showed a more obviously curative effect on all factors of depression after treatment when compared to drug group and acupuncture group.(3) Acupuncture based on combination of Yuan and Luo points method and cognitive therapy can significantly improve the level of NE、DA and5-HT in patients’ blood plasma.
