| Background:Wan Shou-yung, veteran doctor of TCM, born in 1924 in Xinyan County of Henan Province, he was the third generation of medicine and studied the Chinese traditional medicine skills from childhood and graduated from Kaifeng Normal College. He had proficient studies of Chinese ancient civilization, understanding oracle and read several ancient books about traditional Chinese medicine. Integrating family learning and the ancient wisdom, he set up a TCM pharmacy named Wan Sheng Tang in his hometown and practiced medicine to save people in 1948. Then he moved with the army to Taiwan and practiced medicine for more than seventy years, during which he accumulated a lot of clinical experience of diagnosis and had unique opinion to the treatment of the gynecology and other diagnosis. He wrote many medical books and opened classes with students spread around the world.He thought the treatment of traditional Chinese medicine focused on pulse-taking, and in light of the difficulties for current doctors to learn the orthodox pulse-taking techniques which had gradually lost, therefore, he spread the techniques of pulse-taking from his family to other people, and established a pulse condition-symptoms-channel tropism-prescription way of diagnosis and treatment, so as to inherit the artistry of traditional Chinese medicine.Objectives:This paper is based on the doctor’s analysis method of clinical experience and academic thought inheritance research, through Mr. Wan’s literature research of the gynaecological pulse method and pulse condition-symptoms-channel tropism-prescription way of diagnosis and treatment, in order to inherit his academic thoughts and clinical experience of diagnosis and treatment. This paper hopes that through the study he left to verify those thought and method, so as to offer more experience in clinical diagnosis and treatment to inherit his all experience.Methods:Through Mr. Wan’s books, teaching contents, long-term clinical experience, this paper inducts his academic thought and experience of diagnosis and treatment. In clinical study part, from the diagnosis and treatment sheets he left, we screened a total of 407 pieces which related to the disease of department of gynecology, analyze and conclude every common pulse condition, symptoms, channel tropism and prescription in various situations to construct the diagnosis and academic thought of Mr. Wan.Ach i evements:Through the collection of Mr. Wan’s books, teaching contents, long-term clinical experience, we sort out his origin of academic thought, development, deep exploration on gynecological pulsing methods, inducted clinical diagnosis and treatment experience. His academic thoughts included the theory of generation-inhibition in five elements and attaining a state of harmony, the nourishing of congenital kidney state and the supplement of spleen-stomach condition, pulse-taking is the priority among the four processes, channel tropism, the combination between traditional Chinese medicine and western theories and the inheritance and clinical application of gynecological diagnosis and treatment pulsing methods.As for medical cases arrangement, through the induction and classification of common gynecologic diseases appeared on the treatment sheet, we sort out over 20 diseases, such as dysmenorrhea, pre-menstruation, post-menstruation, symptoms of menstrual cycle, infertility, ovarian tumor, breast cancer and so on through the arrangement of ratio, related symptoms, identification and medicines in common use of each cases if pulse is described. Take dysmenorrhea as example, individual cases which have tight pulse between Zuoguan and Chi are over 96%, with equivalent symptoms:chilly, leucorrhea is much and thin, stomachache and lumbar and pedal pain, etc. Most of them are cold pains. The second one is that there are 77% of the patients whose weak pulse is at Zuoguan liver channel, at the left and right of kidney channel are respectively 92%and 88%. At liver channel, the situation belongs to blood-insufficiency, liver depression or blood stasis, under which the patients often have much blood clot during menstruation, hypomenorrhea or dysmenorrhea. At Chi, it belongs to kidney qi deficiency, whose main symptom is lassitude, and at left Chi, patients often experience frequent micturition. In addition, patients whose kidney channel has deep pulse account for over 50%,50% at right Chi, while 69% at left Chi. The whole symptoms are mostly blood stasis due to cold accumulation, blood-insufficiency and deficiency and cold in kidney. The patients will be prescribed with schizonepetae to remove the exterior cold, the root of three-nerved spicebush, Semen Cuscutae, anise to eliminate interior cold. Take rehmanniae glutinosa, radix paeoniae alba, rhizoma sparganii, peach kernel, safflower, so as to improve blood deficiency and blood stasis, and use eucommia bark, leonurus, Fructus Cnidii and others to nourish weak kidney. Other diseases are analyzed in the same way to have a recommend of sequence of Mr. Wan’s sequence of thought in clinical diagnosis and treatment.Conclusion:1. On the diagnosis and treatment of gynecology, Mr. Wan emphasized the cooperation of internal organs pulse-taking with symptoms and dialectic, medicine matching, compatibility and channel tropism with assistance and guidance to harmonize Yin and Yang. He regarded the condition improvement of liver, spleen and kidney as the first priority of gynecological treatment;2. Through his unique way of dialectic, diagnosis and treatment, he adopted the way of releasing metal to generate water, lumbering wood to protect earth, nourishing liver and kidney and other methods to balance five elements. In accordance with the actual situation of liver and blood stasis status, nourish blood and liver, promote blood circulation to remove blood stasis, care spleen and vital energy and others to make a complete diagnosis and treatment clinical thought of gynecology and also give us a concept of system of traditional Chinese medicine treatment. |
