Study On Government Response Competence Of Major Infectious Disease

Since 21th century, major infectious disease epidemic in China not only seriously threatened lives and health of the public, but did great harm to the economic development and social stability.Being directly responsible for the local public security and social stability as well as leading and coordinating the work of coping with major infectious disease epidemic, the strength of the local government’s coping capability is an important decisive factor to deal with major epidemic.After the event of SARS in 2003, in order to strengthen the weak point exposed in it, local governments started the progress of building an emergency management system whose main content is’one case with three systems’(that is emergency plan, emergency system, emergency mechanism, emergency law). And it also put the construction of the coping ability to major infectious disease epidemic at an important position, but the lack of systematic study results on coping capability cannot offer references.This study starts with the analysis of the characteristics and diffusion mechanism of SARS, bird flu and influenza H1N1 epidemic in China combining with the research results of major public health events coping system at home and abroad. Basing on management theory, it also uses related theories of social medicine, emergency management, statistics and so on. It takes the local government coping ability of major infectious disease epidemic as the study object by using methods of space analysis, concept modeling, questionnaire, statistics analysis to research a series of questions of local governments coping ability.The contents and results of this study include:(1) The study of characteristics and diffusion mechanism of major infectious disease epidemic. Doing space analysis by Moren’s I idex, it describes the characteristics of major infectious disease epidemic according to which to sum up the diffusion mechanism of major disease epidemic. The diffusion mechanism exposes that the relationship between the diffusion power of major infectious disease epidemic, the main tasks to cope with, and required capability is:if the diffusion power of major infectious disease epidemic during every period is different, it requires different local government coping ability to finish main tasks during this period.(2) The study of the elements conceptual model of local government’s coping ability on major infectious disease. Basing on the relationship between the diffusion power of major disease, the tasks to cope with and the required capability, and combining with main problems existing in Chinese public health emergency response system, that is, the existence of’stressing saving and ignoring preventing’ thought in some degree, the shortage of prevention, the lack of authority comprehensive professional departments to deal with emergency public events and so on, it puts forward the elements conceptual model of local government’s coping ability which includes four types of macro ability such as coping preparation, warning management, emergency treatment, post-epidemic treatment and 23 micro elements, namely plan formulated and emergency supplies reserves, permanent organization functions and organizational system, health education, plan drills, plan update,’stressing saving and ignoring preventing’ thought, professional training, emergency system construction, early warning management system, warning work, control counter measures selection, decision-making reference opinions, epidemic information release speed, linkage between departments, emergency decision-making executive, medical teams rescue speed, the consciousness of linkage effect of leadership, emergency evaluation work, emergency management, the government’s prevention on secondary events.(3) The empirical research of the elements conceptual model of local government’s coping ability on major infectious disease. By the empirical analysis of the coping effects of major infectious disease epidemic, local governments coping ability of major infectious disease epidemic includes:plan formulated and emergency supplies reserves, permanent organization function, ability to set up organization system; warning management ability mainly includes the construction of early warning management system ability; the emergency treatment ability mainly includes emergency decision-making, rapid information release information, department joint action, emergency decision-making executive ability; epidemic disease post-processing ability including governments’prevention capacity to secondary events. And through the expert opinions, partial coping ability elements are tasted, the results shows that the model has a good explanation power.(4) The study of problems existing in local governments coping ability today. To explain variable coefficient from the regression model of local governments’concrete coping behaviors and coping effects, that is, the regression coefficient of concrete elements of coping ability, to find that organization system construction, early warning management system construction and the outbreak of the rapid release information ability needs to improve. By considering the effects and present condition of the major infectious disease outbreak coping ability specific components it puts forward plan drills, plan update,’stressing saving and ignoring preventing’ thought the thought and early warning management system building ability also need to gradually improve.The innovations of the study include:(1) By applying the geographic information system technology, this study inquires into space and time evolution characteristic of the significant infectious disease epidemic situation and promulgates the proliferation power, the relationship between the governments’primary mission and ability request.(2) It proposes the local government capacity to cope with major infectious disease epidemic models. Basing on the relationship between the diffusion power of major disease, the tasks to cope with and the required capability, and combining with main problems existing in Chinese public health emergency response system, it puts forward the elements conceptual model of local government’s coping ability.(3) It tests the local governments coping ability to major infectious disease by integrating data of governments coping behaviors, coping effects and experts opinions. By analyzing these data orderly and logically from the questionnaire survey it verifies the local government coping ability concept model, and through the expert opinion data it tests the explaining capacity of the model. By analyzing local government coping behaviors data and considering the effects and present condition of the major infectious disease outbreak coping ability specific components, it finds problems existing in the development of local governments’coping ability and proposes suggestions to improve these abilities.
