| Objective:To observe the effect of ErZhi TianGui Granule(ETG) on the expression of key proteins of Fas Channel in granulosa cells(GC),analyse the relationship between the proteins and pregnancy outcome and explore the mechanism of ETG improving it,hoping to supply possible molecula marker for the evaluation of oocyte and embyo.Method:Clinical research:72 infertile women undergoing In vitro Fertilization and Embyo Transfer(IVF-ET) were randomized into treatment group(ETG+IVF,36 cases) and control group(IVF,36 cases).All the cases were treated with standard long proposal to Controlled Ovarian Hyperstimulation(COH).MGCs were collected during oocytes retrieval,and FasL、Fas、Caspase8、Caspase3 were tested with Western Blot and real time fluorescent quantitation polymerase chain reaction(RTPCR).High quality oocyte rate, high quality embryo rate, clinical pregnancy rate et.al were recorded to be compared between the two groups.Animal experiment :180 female Kunming mice of 7 weeks were randomized into Traditional Chinese Medcine(TCM) group(ETG+COH),control group(COH) and normal group. Half mice of each group provided oocytes and GCs after COH while the other half provided embryos.FasL、Fas、Caspase8、Caspase3 in GCs were tested with Western Blot and RTPCR while ATP of oocytes and embryos were tested with luciferase.Results:Clinical research:There are statistical significance between before and after the treatment in the syndrome points fo Kidney Qi-deficiency(P<0.01),and the same between 2 groups after treatment. Gn dose and days of treatment group are less than that of control group(P<0.01),while high quality oocyte rate, high quality embryo rate, clinical pregnancy rate are higher(P<0.01). FasL、Fas、Caspase8、Caspase3 protein and mRNA are all lower in treatment group and pregnant women.Animal experiment: Quantity of oocytes and embyoes of 3 groups turned out to be: TCM group > control group > normal group(p<0.01)while ATP of oocytes, quantity of high quality oocyte and high quality embryo turned out to be:TCM group > normal group > control group(p<0.01).There are no statistical significance in high quality embryo rate and ATP of embyoes between TCM group and normal group,while both the 2 groups are obviously higher than control group. Caspase3 and Fas of TCM group are lower than control group. Caspase3 mRNA and protein are correlated negatively with the ATP of oocyte and embryo.Conclusions:1. The use of ETG in the COH of IVF-ET can improve syndroms of KidneyQi-deficiency and hormone on HCG day, raise high quality oocyte rate, high quality embryo rate and clinical pregnancy rate with no negative effect on the clinial COH.2.ETG can down-regulate Fas Channel which may be the mechanism of ETG function.3. Caspase3 mRNA and protein are correlated negatively with the ATP of oocyte and embryo,thus are inferred to be the objective marker to evaluate oocyte and embyo.4.ETG can improve the activity of oocyte and embryo which maybe the possible mechanism of its improvement. |
