| Backgroundand Objective:A migraine headache repeatedly occurs on one or both sides of the head with the fluctuating pain,which sometimes affects the eyes and teeth.As onset,the pain is severe,while the pain is relieved,patients are the same as the normal.It belongs to the "head wind","migraine","migraine" and other areas in the Traditional Chinese Medicine.The migraine seriously affects the quality of life and psychological mood of patients when the onset.Long-term migraine can induce cardiovascular and cerebrovascular diseases.In ancient literature,the pathogenesis of migraine was mainly divided into two categories: traumatic migraine and internal migraine.The etiology of traumatic headache can be divided into wind,cold,wind,rheumatism and headache.Internal injury headache is mainly caused by the lack of emotion affecting liver and kidney,fatigue body deficiency,and blood stasis due to long-time illness,etc.Phlegm,fire,deficiency and the etiology of blood stasis cause liver yang headache,kidney deficiency headache,blood deficiency headache,phlegm wet headache,blood stasis headache.There is no effective method for treating migraine,and the main treatment is drug therapy and non-drug therapy.When an acute attack of migraine occurs,oral or injection of the corresponding drug is required for pain relief.Prophylactic use of drugs for the treatment of migraine includesβ-adrenoceptor blockers,calcium antagonists,antiepileptic drugs,and antidepressants.The treatment of non-drug therapy of western medicine for migraine mainly involves physical therapy,including peripheral nerve stimulation,hyperbaric oxygen and psychological intervention.Current treatment of Traditional Chinese Medicine for migraine mainlyincludes Chinese internal medicine treatment,acupuncture and massage therapy,and comprehensive treatment of Traditional Chinese Medicine.Among them,acupuncture therapy is recommended by the World Health Organization as one of the migraine headaches.It not only has definite curative effect,but also can avoid dizziness,gastrointestinal reactions,headaches and other toxic and side effects of drugs,and can effectively regulate the body’s role.As a kind of traditional acupuncture therapy,the fire needle has a strong warming effect and has a certain clinical effect on pain.In the clinical application,it also finds that the fire needle can relieve the symptoms of the headache patients.Therefore,this experiment tried to use fire needle as an intervention method to observe the effect of fire on the clinical efficacy and quality of life of patients with migraine on the one hand;on the other hand,to establish a set of scientific evaluation of the efficacy and safety of fire acupuncture for the treatment of migraine.The standardized,clinically effect of migraine of fire needle treatment,providing scientific evidence for the treatment of migraine with fire needle therapy.MethodAccording to the criteria of diagnosis,inclusion,and exclusion,60 migraine patients were screened at the Acupuncture Department of the Second Hospital of the of Guangdong Province.the 60 patients were randomly divided into treatment group and control group,30 cases in each group using random Numbers table and using the envelope method to hide the random numbers..(1)The treatment group received fire needle treatment.The specific methods are as follows:Acupoints: the main points of the acupoints: Touwei,Shuaigu,Yangchi,Quxu,Ashi.Acupoints accordanced with differentiation of syndrome: syndrome of brain blocked by blood stasis: Ashi points,Yushu,Hegu,Sanyinjiao;syndrome of hyperactivity of liver yang: Hanyan,Taichong,Xiaci,Sanyinjiao;syndrome of turbid phlegm obstructing in brain: Zhongwan,Hegu,Fenglong;syndrome of deficiency of both qi and blood: Xinshu,Pishu,Zusanli,Sanyinjiao;syndrome of yin deficiency of liver and kidney: Naokong,Shenshu,Taichong,Taixi.The acupoint positioning is based on the 12 th five-year-plan textbook Acupoints and Meridians edited by Liu Qingguo.Needle tool: Fire needle.Method of operation: The patient takes a supine position or sittingposition.The doctor marks the acupoints bythe nails,and an iodine partial disinfection is performed at the acupoints.After disinfection,apply a thin layer of beaten dandelion oil on the acupuncture point.The doctor holds the alcohol lamp or hemostatic forceps to hold the 95% alcohol tampon in the left hand(pinch dry).The right thumb,index finger,and middle finger hold the fire needle.Fire needle handleis put approximately 3cm above the acupoint,then inclined the front needle body and the needle tip to approximately 2/3of the needle with a tilt of approximately 45°(after the needle body is burnt).Insert the fire needle quickly and vertically with a depth of about 0.2-0.3cm(the length of the firing needle must be greater than the depth of the puncture).After the needle is out,use a sterile dry cotton ball to press the needle hole quickly for a moment to relieve pain and prevent bleeding.Then apply a layer of beaten marigold oil on the needle hole to protect the wound.Inserting depth can be appropriately adjusted according to the patient’s condition of thinning,alternating with bilateral acupoints.Course of treatment: the treatment was performed on the 1st,3rd,and 5th days of each course of treatment,5 days for one course of treatment,and 2days of rest after one course of treatment,for a total of 2 courses of treatment.(2)The control group received traditional Chinese herb decoction treatment.The prescription was as followsIt is based on the decoction of Dabu Yuan Jian,and is added and subtracted with the syndrome.Da Bu Yuan Jian: Dangshen 10 g,Shanyao 15 g,Shudi 15 g,Duzhong 15 g,Danggui15g,Shanzhuyu 15 g,Gouqi 15 g,Zhigancao 6g.With cold and yang deficiency: Rougui,Fuzi,Paojiang;qi deficiency:Huangqi,Baizhu;blood stasis: Chuanxiong and Shanzhuyu subtracted;diarrhea:Wuwei,Buguzhi.Add water 400 ml decoction to 150 ml,one dose per day,drink after meal.Course of treatment: 5 days a course of treatment,2 days after a course of treatment,a total of 2 courses.Observe and record the relevant test data before treatment,1 week,and2 weeks after the observation period with 2 weeks,including basic patient data,symptom scores using a headache score scale,and Migrainew Specific Quality of Life(MSQ),McGill Pain Questionnaire(MPQ),migraine-specificquality of life questionnaire.To assess the quality of life,a simple McKinley Pain Questionnaire was used to assess the degree of pain,and the clinical efficacy was evaluated by the Nimodipine method.Data analysis was performed by statistical software SPSS15.0.Measured data were expressed as mean±standard deviatio,and count data were expressed as composition ratio(%);measurement data sets were compared using two independent sample t-tests(rank of variance was used for the rank sum test),and they were used before and after comparison.Paired t test or Wilcoxon paired rank sum test(unequal variance).The chi-square test was used for the comparison among the categorical data sets,and the rank sum test was used for the comparison between the data sets.Multivariate comparisons were performed using one-way ANOVA(One-Way AN0VA)tests and after homogeneity of variance tests,multiple comparisons were performed using the LSD method.P<0.05 was the significant difference standard of all statistical analysis.Result1.Baseline comparison: Before treatment,general information such as age,gender,and course of disease of the two groups were statistically analyzed.There was no significant difference between the two groups(P>0.05).There was comparability between the two groups.Headache rating scales were scored before treatment,as well as MSQ scores and the MPQ score.After statistical tests,there was no statistical difference between the two groups of patients before the score of each scale(P>0.05).The data between the two groups were comparable.2.Headache symptom scores: The scores of headache symptoms were significantly decreased after two courses of treatment in both the fire needle group and the traditional Chinese medicine control group,and there was a curative effect in alleviating pain.There was a statistically significant difference in efficacy between the two groups after treatment(P<O.05).Fire needle therapy for migraine is superior to traditional Chinese medicine in reducing pain in patients.3.Quality of life scores: MSQ scores increased significantly after two courses of treatment in both groups.Fire acupuncture and traditional Chinese medicine therapy were effective in improving the quality of life of patients.There was a statistically significant difference in efficacy between the two groups after treatment.(P < O.05),indicating that fire acupuncture treatmentof migraine is better than traditional Chinese medicine in improving the quality of life of patients.4.Pain degree score: The SF-MPQ scores decreased after two courses of treatment in both groups,that is,fire acupuncture therapy and Chinese medicine treatment migraine are effective in reducing the degree of headache.The difference in efficacy between the two groups after treatment is statistically significant.The significance(P < 0.05)indicates that the fire acupuncture treatment of migraine is better than traditional Chinese medicine in reducing the degree of headache.5.Clinical total effective rate: The total effective rate of fire acupuncture therapy and traditional Chinese medicine therapy for migraine reached 86.7% and 76.6%,respectively,and the total effective rate between the two groups was statistically significant(P < 0.05).The total effective rate of fire acupuncture therapy for migraine is better than traditional Chinese medicine therapy.ConclusionThe above data all indicate that the fireneedle acupuncture therapy and the Chinese herb decoction therapy can also effectively reduce the pain,and the times of episodes,shorten the headache duration,improve the accompanying symptoms and improve the quality of life.The fire needle acupuncturetherapy in the headache symptom score scale,migraine-specific quality of life questionnaire,MSQ,MPQ,and the clinical therapeutic efficacy indicators were all superior to those of the Chinese herb decoction group.Fire needle acupuncturetherapy had obvious advantages for migraine treatment.As a accient therpy of TCM,the efficacy of fire needlefor the treatment of migraine is correct,and the therapy is safe and has no side effects.It provides a new treatment plan for the clinical treatment of migraine and provides scientific evidence for the treatment of migraine with fire needle acupuncture. |
