ObjectiveGall disease is a kind of neck disease characterized by anterior cervical enlargement,which refers to thyroid disease in modern medicine.Although the ancient books of traditional Chinese medicine have accumulated rich and valuable clinical experience for the treatment of gall disease,there is no systematic summary of the guiding role for the treatment of single thyroid disease."All kinds of diseases of the six meridians",but at present,the academic research on the differentiation and treatment of the six meridians is still insufficient.Based on the basic research of gall disease in ancient Chinese medical literature,combined with the literature research of modern Chinese medical records,clinical research materials and retrospective study of clinical cases,this subject completed a large-span research from ancient to modern,from theory to clinical practice,clarified the source and course of gall disease treatment,and combed the treatment experience of contemporary physicians on six meridians,based on meridians,viscera and so on.Taking hyperthyroidism as an example,this paper summarizes and extracts the characteristics of symptoms and pathogenesis of the six meridians syndrome,and incorporates the time into the six meridians syndrome differentiation system to enrich the content of the six meridians syndrome differentiation system,which is the beginning of literature research on the six meridians syndrome differentiation and treatment of gall disease,and also provides methodological experience for future research on the six meridians syndrome differentiation and treatment of complex diseases.MethodsThis topic is divided into five parts.The first part is a comprehensive review of the silk book unearthed from Mawangdui to the present modern Chinese medicine literature,and a literature review is completed to explore the connotation of the six meridians syndrome differentiation,and systematically summarize the characteristics of the differentiation and treatment of gall disease by physicians of past dynasties;the second part is based on the principle of frequency statistics and mathematical attribute partial order through the collection of ancient Chinese medicine books.Systematically summarize the prescriptions and medicines for gallstones of doctors in past dynasties,summarize the characteristics of medication,meridian tropism and the rules of prescription composition;Part three and Part four collect the modern Chinese medicine literature in China national knowledge internet and Wanfang database,as well as the effective prescriptions and medicines for hyperthyroidism and related symptoms and pathogenesis,respectively,and apply the theory of differentiation of six meridians based on meridians and viscera.The typical symptoms and pathogenesis of six meridians syndrome of hyperthyroidism were analyzed by disordered multi-classification logistic regression method.Combining with the principle of partial order of mathematical attributes,the prescription rules of hyperthyroidism treatment drugs were excavated.The characteristics of symptoms,pathogenesis and prescriptions of six meridians differentiation and treatment of hyperthyroidism were summarized,and a symptoms-pathogenesis was proposed.Complete six meridians syndrome differentiation system of hyperthyroidism.The fifth part,in-depth discussion of ancient gall disease treatment experience on the treatment of hyperthyroidism guiding role and based on the six meridians syndrome differentiation system of hyperthyroidism syndrome characteristics.Results1.The term "Six Classics" originated from the Internal Classics.Through the interpretation of physicians of past dynasties,it is believed that the Six Classics of Shanghan Lun are based on the physiological and functional basis of meridians and zang-fu organs,including the twelve Classics of Hand and Foot and their related zang-fu,qi,blood,Yin and yang,body fluid,etc.It has become a consensus to understand the six-channel syndrome differentiation system of Shangfen Lun from the perspective of comprehensive concepts.Although the typical symptoms of gall disease are confined to the anterior cervical region,the systemic lesions of gall disease mostly involve viscera and meridians,which can be classified according to the six meridians location and integrated into the six meridians syndrome differentiation system.Modern physicians use six meridians syndrome differentiation and classical prescriptions to treat gall disease(thyroid disease),including hyperthyroidism,hypothyroidism and thyroiditis,which have great therapeutic advantages.2.The typical symptoms of gall disease in ancient Chinese medicine books are anterior cervical enlargement.The associated symptoms are mostly concentrated in throat and heart and chest,such as throat swelling,heart and chest disadvantage,etc.The description of systemic accompanying symptoms is less.The cause of disease is mostly around six pathogenic factors,water and soil disorders,organ dysfunction,emotional disorders,physical differences,appropriate regulation,etc.,and the related pathogenesis is more dispersed,the organs mostly involve the liver and spleen.The efficacy of traditional Chinese medicine is mostly phlegm-resolving drugs,heat-clearing drugs,tonifying deficiency drugs,Qi-regulating drugs,and the main taste is cold,bitter and bitter.According to the golden division method of meridian tropism of traditional Chinese medicine,it can be roughly inferred that the distribution of the six meridian syndromes is too Yin-Shaoyin syndromes,too Yin-Jueyin syndromes,Yangming-Taiyin syndromes,Taiyin syndromes,Taiyin-Jueyin syndromes and Shaoyin syndromes.The main medicines for gall disease in ancient books of traditional Chinese medicine are algae,kumbu,clamshell,pineapple,Pinellia ternata,gentian and other phlegm and heat-clearing drugs.3.Through the study of 434 cases of modern Chinese medicine,139 symptoms related to hyperthyroidism were obtained.In addition to palpitation,cervical thickness,hand tremor and emaciation,the more accompanying symptoms were eye distension,dizziness,fever,spontaneous sweating,dry mouth,bitter mouth,irritability,insomnia,distress,less menstruation,late menstruation,thin stool,diarrhea and yellow urine.Common tongue and pulse signs include red tongue,less fur,thin yellow fur,yellow fur,fine pulse,fine pulse,fine pulse,fine pulse,fine pulse,fine pulse,etc.A total of 32 related pathogenesis were obtained.Common pathogenesis were liver depression,liver-qi depression,hyperactivity of liver-fire,disturbance of liver-yang,Yin deficiency,phlegm-heat,deficiency of both qi and yin,phlegm-blood stasis and phlegm-qi interaction.The commonly used Chinese medicines are mainly tonifying deficiency,clearing away heat and resolving phlegm-The medicinal properties are mostly cold Chinese medicines,with more bitter and bitter Chinese medicines.Typical syndromes of six meridians are Yang-ming-heat,Qi-yin injury,Shaoyang dysfunction,internal depression of gallbladder(liver)fire,Taiyin-spleen deficiency,internal excess of cold and dampness,Shaoyin-heat,Shaoyin-cold,Jueyin-heat-up and cold-down.4.Through a retrospective study of 249 cases of clinical cases,122 symptoms related to hyperthyroidism were obtained.In addition to the typical symptoms of palpitation,cervical thickness,hand tremor and emaciation,more accompanying symptoms were skin itching,sweating,nausea,cold,dry mouth,bitter mouth,epilepsy,narcolepsy,thirst and excessive drinking,insomnia,fatigue,fatigue,dreaming,anxiety,irritability,irritability and irritability.Less menstruation,thin stool,etc.Common tongue and pulse signs include red tongue,light tongue,light red tongue,thin white fur,white fur,fine fur,deep vein,stringed vein,stringed vein and so on.A total of 35 prescriptions were obtained,including Chaihu Guizhi Decoction,Xiaochaihu Decoction,Guizhi Decoction and Lizhong Decoction.Typical syndromes of six meridians are Yang meridian,Yang-Ming meridian,Shaoyang meridian,Taiyin meridian,shaoyin-cold syndrome,jueyin-cold-heat syndrome,jueyin-heat syndrome,jueyin-cold syndrome,mid-Jiao-cold-heat syndrome,sun-shaoyang syndrome and sun-taiyin syndrome.5.Through the retrospective study of medical records,clinical research literature and clinical cases,the six meridians syndrome types of hyperthyroidism are sun wind,Ying-wei disharmony,Yang-Ming heat,Qi-Yin injury,Shaoyang dysfunction,liver(gallbladder)fire depression,Taiyin spleen deficiency,cold-dampness syndrome,shaoyin-fever syndrome,shaoyin-fever syndrome,jueyin-fever-heat syndrome,jueyin-fever syndrome,jueyin-fever syndrome,jueyin-fever syndrome,jueyin-cold syndrome and the mixed syndrome of cold and heat in Zhongjiao.Conclusion1.Combining with the characteristics of gall disease treatment in ancient books of TCM,the guiding role of gall disease in the treatment of hyperthyroidism was analyzed.Hyperthyroidism is not suitable for the use of seaweed,kumbu,clamshell and other iodine-containing traditional Chinese medicines and animal thyroid products,but other non-iodine phlegm-resolving drugs,such as pineapple,Pinellia ternata,fritillaria,and with forsythia,gentian,white clam and other antipyretic drugs,cinnamon,ginseng,angelica,roasted licorice and other tonic drugs,tangerine,wood incense,betel nut,green peel and other Qi-Regulating drugs.This is the case.2.Wind pathogens affect the onset and clinical manifestations of hyperthyroidism.In view of the characteristics of hyperthyroidism,such as hand tremor,tongue tremor,skin itching,headache and eye diseases,wind pathogens correspond to the pathogenic characteristics of active,unstable,active,numeric change and light,rising,upward and outward,suggesting that wind pathogens exist in the pathogenesis of hyperthyroidism.By studying the prescription law of modern Chinese medicine literature and clinical cases in treating hyperthyroidism,from the point of view of pathogenic wind,skin itching symptoms are mostly seen in the sun meridian syndrome or combined with the sun meridian syndrome,the sun meridian syndrome is not in harmony with defense,wind pathogens invade and Qi and blood imbalance.Wind medicine such as Fangfeng,Tribulus terrestris,Brassica juncea spike,Ligusticum Chuanxiong and so on can be used in the treatment.Eye diseases mostly occur in the syndrome of Shaoyang meridian,and the essence of liver is noticed in the eyes along the meridian.The meridians of hand and foot are connected with the canthus to nourish the orifices of liver.Therefore,the normal function of the purpose depends on the flow and transmission of the meridian of Shaoyang.When the syndrome of Shaoyang meridian is accompanied by eye diseases,wind medicines such as bupleurum,platycodon,forsythia,burdock,summer withered grass can be used.3.Hyperthyroidism has diarrhea,fatigue,fatigue,edema and other symptoms of deficiency and related pathogenesis,especially elderly hyperthyroidism.Deficiency symptoms such as loss of appetite,diarrhea,fatigue,fatigue and edema are closely related to the secrets of deficiency diseases such as Qi deficiency,Yang deficiency and Yin deficiency.Despite the existence of the pathogenesis of hyperthyroidism,not all patients will appear symptoms of deficiency,and most of them are still mixed with deficiency and excess.In the pathogenesis of hyperthyroidism,Qi deficiency,Yin deficiency or Qi-Yin deficiency appeared earlier.With the progress of the disease,Yin deficiency became more and more serious,Yin damage and deficiency of Taiyin and spleen-yang,deficiency of Taiyin and spleen-qi,deficiency of Shaoyin and kidney-yin,deficiency of Shaoyin and kidney-yin,deficiency of both Yin and Yang,deficiency of spleen and kidney-yang of Taiyin and Shaoyin syndromes,etc.Therefore,senile hyperthyroidism with indifferent symptoms can be treated from the Taiyin and Shaoyin meridians.4.In addition to liver,the pathogenesis of hyperthyroidism still involves spleen,kidney,heart,lung,stomach,gallbladder and other viscera.Hyperthyroidism is treated from the liver,the common pathogenesis is hyperactivity of liver fire,stagnation of liver depression and qi,and internal movement of liver wind.The common symptoms are irritability,fear of heat,blindness,emotional depression or anxiety,chest tightness,good breathing,hand tremor,eye astringency.Hyperthyroidism is treated from the spleen theory,and the common pathogenesis of Spleen-Yang deficiency is manifested in the clinical manifestations of fatigue,fatigue,dementia,diarrhea and so on.Manifestations:hyperthyroidism is treated from the kidney,and the pathogenesis of kidney yin deficiency,Kidney Yang deficiency,Yin and yang deficiency are common.Common symptoms include cold limbs,mental fatigue,weak waist and knee,cold limbs,soreness of waist and knee,abdominal cold,diarrhea,edema of limbs,dizziness,tinnitus,weakness,palpitation,restlessness,insomnia,palpitation,restlessness,insomnia,dreaminess,etc.Hyperthyroidism is treated from the lung,and is closely related to phlegm,such symptoms as palpitation,chest tightness or pneumonia.Damage affects the operation of heart and blood,such as palpitation,sweating,fatigue and other symptoms of Qi-Yin deficiency;hyperthyroidism is treated from the stomach,the key pathogenesis is gastric fire,gastric fire or stomach yin deficiency;hyperthyroidism is treated from gallbladder,often accompanied by emotional symptoms,such as anxiety,depression,and easily frightened-Hyperthyroidism is characterized by deficiency of gallbladder and qi.Hyperthyroidism is not only treated from the liver,but also involves the spleen,kidney,heart,lung,stomach,gallbladder and other viscera,which also lays the foundation for the differentiation of six meridians.5.The Taiyang meridian syndrome of hyperthyroidism is caused by solar wind and incompatibility between Ying and Wei.Typical symptoms include sore throat or headache,nausea or chills,itching of skin,shoulder and arm pain,etc.Guizhi Decoction is often used in the treatment of hyperthyroidism.Yangming meridian syndrome of hyperthyroidism is caused by Yangming stomach heat and Qi-yin injury.Typical symptoms of hyperthyroidism are alleviating cereal and hunger,emaciation,exophthalmos,nausea,thirst and excessive drinking.White tiger and ginseng decoction are often used in the treatment of hyperthyroidism.Shaoyang Channel Syndrome of Hyperthyroidism is characterized by loss of center of Shaoyang and depression of liver(gallbladder)fire.Typical symptoms include headache,distention of eyes,blush of eyes,bitter mouth,chest tightness,chest and hypochondriac distention,urgency,irritability,sigh of joy and anxiety.Chaihu Shugan San or Xiaochaihu Decoction are often used in the treatment of hyperthyroidism.Taiyin meridian syndrome of hyperthyroidism is caused by spleen deficiency of Taiyin and excessive cold and dampness.Typical symptoms include dullness,edema of lower limbs,fatigue,fatigue,diarrhea,thin stool and so on.The pathogenesis of hyperthyroidism is Yin deficiency,heart-liver-yin deficiency or Qi-yin deficiency.The typical symptoms are palpitation,hand-foot-heart fever,insomnia or restlessness at night.The therapeutic agents are Shengmaisan,Tianwang Buxindan and Danggui Liuhuang Decoction.The pathogenesis of hyperthyroidism is deficiency of spleen and kidney yang.Typical symptoms include dizziness of eyes,fatigue,fatigue,thin stool and so on.Zhenwu Decoction and Fuzi decoction are commonly used in the treatment of hyperthyroidism.At the same time,indifferent hyperthyroidism can be treated from the syndrome of Taiyin meridian and the syndrome of shaoyin-cold.The syncope of hyperthyroidism is Yin-cold-heat syndrome with the pathogenesis of upper heat and lower cold syndrome.Typical symptoms include dry throat,thirst,excessive drinking,stasis,fatigue,fatigue,less menstruation,diarrhea and so on.Xiaoyaosan,Sinisan and Wumei pills are commonly used in the treatment of hyperthyroidism.Jueyin-heat syndrome of hyperthyroidism is caused by damp-heat of the liver meridian.The typical symptoms are heat-benefit,acute and severe,swelling and pain of the ribs and flanks.Baitouweng Decoction is used to add or subtract the prescription.Jueyin-cold syndrome of hyperthyroidism is caused by cold coagulation of liver meridian.Typical symptoms include cold hands and feet,headache,retching,salivation,etc.The prescription can be added or subtracted by Angelica Sini plus Evodia and Ginger Decoction.The typical symptoms of the syndrome are thirst,excessive drinking,abdominal distension and irregular stool knots,which can be added or subtracted by Banxia Xiexin Decoction.
