Objectives:The China Cardiovascular Report 2018 pointed out that hypertension is the most common chronic non-communicable disease and the disease with the largest burden of disease in the world[1].Controlling the occurrence and prevalence of hypertension is of great significance.There are many studies on the control of hypertension and health management at home and abroad,but most of them are established according to the characteristics and conditions of the medical systems of other countries,or have a certain focus.Most of the implementation in China cannot comprehensively control hypertension.And management,so this study explores the establishment of a chronic disease health management model suitable for China's national conditions,and at the same time based on this model to evaluate the effects of interventions in patients with hypertension.Materials and methods:In this study,from July 2013 to August 2013,the cluster random sampling method was used to investigate the influencing factors of hypertension in high-risk groups of chronic diseases in Shenyang City.A cross-sectional study was conducted to select 3,014 permanent residents over 18 years of age in urban communities in Shenyang City.The survey respondents filled out the questionnaire face-to-face,collected 3014 valid questionnaires,and the effective recovery rate was 100%.This study obtained data through questionnaire surveys and physical examinations,including questionnaires including health risk assessment questionnaires,anxiety self-assessment scale?SAS?and depression self-assessment scale?SDS?,using SPSS 22.0 for statistical analysis and univariate analysis Use thec2 test,multivariate analysis using Logistic regression analysis;secondly,draw on domestic and foreign research innovations to build a new integrated health management model and apply it to conduct intervention studies.The intervention studies randomly selected 27 community health services in five administrative regions in Shenyang A total of 812 18-79-year-old hypertensive patients visited the center for intervention of hypertensive patients from September 2013 to September 2015,of which346 people in the experimental group adopted an integrated health management model and466 people in the control group adopted traditional health management Model,both groups took a prospective cohort study,obtained data through questionnaires and physical examination,and finally completed 680 people with continuous management,with an overall loss of follow-up rate of 16.3%,including 284 in the test group and 396 in the control group,intervention study SPSS22.0 was used for statistical analysis,and thec2test was used to test the change of rank,and the higher the rank was,the higher the progress was.The same grade is regarded as"no change",and the lower grade is regarded as"improvement";repeated measurement analysis of variance is used for continuous variables to study the changes in blood pressure and other secondary indicators of hypertension patients with different intervention methods and different intervention times Impact,categorical variables are analyzed by generalized estimation equations,and the test level is?=0.05.Result:This study first conducted a cross-sectional survey to investigate the biological indicators,behavior indicators,psychological levels,and coexistence of various chronic diseases in the community of high-risk groups of chronic diseases in Shenyang's urban communities,and obtained the high-risk groups of chronic diseases in Shenyang's urban communities.Risk factors for hypertension are age,income level,family history of hypertension,BMI,fasting blood glucose,total cholesterol,cooking oil intake,and combined hyperlipidemia;secondly,this study draws on advanced experience in health management at home and abroad,using literature research methods and special topics The group discussion method was the first to build a new integrated health management model based on evidence-based medicine,personalized,comprehensively integrated general practice and personalized disease prevention.Next,based on the new model constructed to carry out intervention studies of patients with hypertension.The study applied a new integrated health management model and traditional health management model to two-cycle interventions in patients with hypertension,and obtained?1?The overall two-cycle progression rate of systolic blood pressure in the experimental group was lower than that in the control group.In the first cycle,the progress rate of systolic blood pressure in middle-aged and elderly hypertensive patients was lower in the test group;in the second cycle,the progress rate of systolic blood pressure in the three age groups was lower than that in the control group.Throughout the two cycles,the comparison between the experimental group and the control group before and after found that there was a difference in systolic blood pressure before and after the intervention?P<0.05?,and there was an interactive effect between the time point and the intervention method,that is,the experimental group and the control group were in Systolic blood pressure changes before and after.?2?The overall diastolic blood pressure in the test group and the control group showed no significant difference in the entire intervention period,but the overall trend of the test group and the control group was declining.In the second cycle,it can be seen that the improvement rate of intervention of young people is much larger than that of the control group.Throughout the two cycles,the diastolic blood pressure of the test group and the control group before and after the intervention was different before and after the intervention?P<0.05?,the time point can significantly affect the change of diastolic blood pressure,that is,the diastolic blood pressure of the patients in the test group and the control group have different trends at different intervention time points.?4?Through two cycles of intervention,the blood glucose,blood lipid and behavior-related indicators of the test group and the control group have improved to varying degrees.Conclusions:In this study,the risk factors for hypertension in the high-risk groups of chronic diseases in urban communities in Shenyang were age,income level,family history of hypertension,BMI,fasting blood glucose,total cholesterol,cooking oil intake,and combined high blood fat;Advanced management experience,the first to innovate and build a new integrated health management model,comprehensive integration of general practice and personalized disease prevention.The new integrated health management model was used to intervene in patients with hypertension for two cycles,and the new integrated health management model was found to be practical and effective,which was superior to the traditional health management model.
