An instance of knowledge representation: Measuring the domain completeness of the nursing interventions classification system in parish nurse documentation

As the health care industry moves toward computerizing the health care record, health care professionals have been challenged to capture their domain of practice in a computer-friendly format. Nursing has answered that call by developing nursing standardized vocabularies. This study measures the domain completeness of one vocabulary, the Nursing Interventions Classification (NIC) in parish nursing, a community-based practice. The sample included 1607 interactions from 170 health records submitted by 13 parish nurses in 7 sites across the country, stratified by urban, suburban, and rural sites. Nursing interventions from the documentation were mapped into NIC using standardized mapping procedures.; Study results demonstrate that it is possible to reliably map parish nurse documentation into NIC and that NIC generally supports the documentation of parish nursing practice. Parish nurses perform significantly higher number of interventions in the BEHAVIORAL, SAFETY, and HEALTH SYSTEM Domains and significant differences appeared based on urban, suburban, and rural sites. Overall, 93% of the 3059 interventions mapped into NIC. Of the interventions that could not be mapped, interventions related to administrative interventions specific to the community, to current NICs that required a community focus, and to new concepts not captured in NIC. The study also supported the ability of NIC to capture the spiritual dimension of care. Findings support the use of NIC in parish nursing with some modifications.
