BackgroundAs clinical markers for the detection of malignant tumors,serum tumor markers are widely used in clinical practice due to their low cost,non-invasive,simple and repeatable management.At present,there are markers such as CEA,CA19-9,CA125,CA72-4 and AFP etc.which are very closely related to gastric cancer(GC).MethodsFrom January 2015 to January 2016,the clinical data and serum tumor markers value of 376 GC patients undergoing surgery at the Shanghai Jiao Tong University School of Medicine affiliated Ruijin Hospital Department of Gastrointestinal Surgery were collected and the levels of expression of CEA,CA19-9 and CA12-5 in the tumor tissues of a random sample of 99 cases were detected by immunohistochemistry.The relationship between the expressions of the serum and tumor tissue markers and the clinicopathological characteristics of the patients and the correlation between the serum and tumor tissue tumor markers were separately investigated.ResultsIn this group of GC patients,the positive rates of serum CEA,CA19-9,CA125,CA72-4 and AFP were 19.41%,17.82%,7.71%,14.89% and 3.99% respectively.The level of serum CEA was closely related to the tumor histological type(P<0.01),depth of tumor invasion(P<0.05),TNM staging(P=0.001);The level of serum CA19-9 was closely related to age of the patient(P<0.001),maximum tumor diameter(P<0.001),depth of tumor invasion(P<0.001),lymph node metastasis(P<0.05),distant metastasis(P<0.001),peritoneal metastasis(P<0.001),ascites(P<0.001)and TNM staging of the tumor(P<0.05);The level of serum CA125 was closely related to maximum tumor diameter(P<0.001),histological type(P<0.05),depth of tumor invasion(P<0.001),lymph node metastasis(P<0.01),distant metastasis(P<0.01),peritoneal metastasis(P=0.001),ascites(P<0.001)and TNM staging of the tumor(P<0.001);The level of serum CA72-4 was closely related to the maximum tumor diameter(P<0.001),depth of tumor invasion(P<0.01),distant metastasis(P<0.001),peritoneal metastasis(P<0.001),ascites(P<0.001)and TNM staging(P<0.01);The level of serum AFP was not significantly related to any of the above mentioned clinicopathological characteristics of the patients.The rates of positive expression of the tumor tissue CEA,CA19-9 and CA125 were 62.63%,42.42% and 10.10%,respectively.The level of tumor tissue CEA expression was related to the maximum tumor diameter(P<0.05),depth of tumor invasion(P<0.01)and TNM staging(P<0.05);The level of tumor tissue CA19-9 expression was related to tumor invasion depth(P<0.01)and TNM staging(P<0.05)while the level of tumor CA125 expression was not related to any of the clinicopathological characteristics of the patients.The serum and tumor tissue expressions of CEA,CA19-9 and CA125 were significantly correlated(P<0.01).ConclusionThe clinical significance of serum tumor markers in the diagnosis of early stage GC is limited,but has displayed certain value in the evaluation of preoperative clinical staging,especially the diagnostic role of serum CA125 in the evaluation of peritoneal metastasis and ascites which however requires further assertion.The serum CEA and CA19-9 are mainly synthesized and secreted by the tumor tissue while CA125 mostly originates from peritoneal mesothelial cells.The level of serum tumor marker is not only related to the quantity produced,but is also affected by the secretory pathway of the tumor markers.
