Part ?Objective To explore the distribution characteristics of cognitive function in patients with obsessive compulsive disorder(OCD)and their unaffected first-degree relatives(UFDR).Methods A total of 48 patients with OCD,41 UFDR and 32 healthy controls(HC)were enrolled.The cognitive function was tested by the MATRICS Consensus Cognitive Test(MCCB).Results 1.Compared with the HC group,the scores of the trail making test and the continuous operation were lower in the OCD group than those of the HC group,and there were statistical differences between the two groups(p<0.05);The scores of the symbol encoding,the verbal memory,the spatial span,the maze test,the visual memory and the verbal fluency were lower than those of the HC group,and there were no statistical differences between the two groups(p>0.05).2.Compared with the HC group,the scores of the symbol encoding,the verbal memory,the visual memory,the spatial span and the verbal fluency in the UFDR group were higher than the HC group,but the scores of the maze test were lower than those of the HC group,and there were no statistical differences between the two groups(p>0.05).and the score of the trail making test and the continuous operation were lower than those of the HC group too,and there were statistical differences between the two groups(p<0.05);3.Compared with the UFDR group,the score of the verbal fluency in the UFDR group were higher than those of the OCD group,and there was a statistically difference between the two groups(p<0.05);The scores of the trail making test,the symbol encoding,the verbal memory,the spatial span,the maze test,the visual memory and the continuous operation in the OCD group were lower than those of the UFDR group too,and there were no statistical differences between the two groups(p>0.05).Conclusion 1.The cognitive function was tested by MCCB,the patients with OCD have cognitive impairment,mainly manifested in attention concentration and maintenance;2.The cognitive function was tested by MCCB,the cognitive impairment was found in the unaffected first-degree relatives,mainly manifested in attention concentration and maintenance too,these characteristic changes may be the endophenotype for OCD;3.The cognitive function was tested by MCCB,the study did not show the following distribution characteristics of cognitive function among three groups(from severe to mild):the cognitive function impairment in the OCD group was more severe than that of the UFDR group,and the cognitive function impairment in the HC group is the mildest among three groupsPart ?Objective To investigate the characteristics of resting-state functional magnetic resonance imaging in the patients with OCD and their unaffected first-degree relatives.Methods A total of 52 patients with OCD,35 UFDR and 38 HC were enrolled.Then the resting-state fMRI scan was conducted for each participant.The amplitude of low frequency(ALFF),the fractional amplitude of low frequency(fALFF)and Regional Homogeneity(ReHo)was used to explore the intensity of brain function in the different brain regions under resting-state among the three groups.The function connection(FC)was performed using the brain regions with significant differences in fALFF and ReHo as the seed points.Results 1.There was no statistical difference in ALFF among the three groups(p>0.05);2.There were statistical differences in fALFF of right cerebellum_Crus2,right inferior temporal gyrus,left rectus gyrus and left caudate among the three groups(p<0.05);3.There were statistical differences in ReHo of left cerebellum(area 9),left inferior orbital frontal gryrus,left insula(2 area),right angular(2 area)and left inferior parietal gyrus among the three groups(p<0.05);4.Compared with the HC group,using with insula,inferior parietal gyrus and putamen gyrus as the seed points,the intensity of the FC in the OCD group in the right pallidus,right middle cingulate gyrus,right superior frontal gyrus,left inferior temporal gyrus(two area),left angular gyrus,right cerebellum area 8 and left cerebellum-Crus1 is decreased,In the left fusiform,right fusiform,left middle temporal gyrus,right middle temporal gyrus,right postcentral gyrus,right superior temporal gyrus,right front cingulate gyrus,left superior temporal gyrus(2 area)and right precentral gyrus,the intensity of the FC is increased;Compared with the HC group,the intensity of FC in left cerebellum(area 9),left putamen left thalamus(2 area)and right superior parietal gyrus is decreased in the UFDR group.The intensity of FC in the right anterior cingulate gyrus is increased.Compared with the UFDR group,the intensity of the FC in the left inferior frontal gyrus,right middle frontal gyrus,right middle cingulate gyrus,left auxiliary motor area and right Parahippocampal gyrus is decreased in the OCD group,the intensity of the FC increased in the left middle occipital gyrus.Conclusion 1.Obsessive-compulsive disorder patients have obvious abnormal brain function activity in the brain regions of CSTC;2.The unaffected first degree relatives have obvious abnormal brain function activity in the brain regions of CSTC too;3.The same changes in FC from insula to striatum and from inferior parietal gyrus to right anterior cingulate gyrus were present in both OCD patients and their unaffected first-degree relatives suggest that impairment may be an endophenotype for OCD.
