Comparison Of Early-stage Inflammatory Responses After Robotic And Laparoscopic Radical Surgery For Colorectal Cancer

Abstract/Summary:
Objective: By comparing and analyzing the perioperative immune indicators of patients having robotic or laparoscopic radical colorectal surgery,this prospective and controlled study aim to describe the differences between robotic surgery and laparoscopic surgery from the perspective of post-traumatic stress and systemic inflammation.Methods: From August 2019 to January 2020,patients diagnosed with colorectal cancer were prospectively included and divided into robotic and laparoscopic groups.Serum inflammation indicators(CRP,IL-6,IL-8,TNF-α,T cell,NK cell,Blood cell analysis)were collected pre-operation and 24 h,72h after surgery.Results: A total of 86 patients were enrolled in the study,including 25 in robot group and 61 in laparoscopic group.Each group was comparable in terms of age,gender,BMI,ASA score,tumor location and cancer stage.The robotic surgery group had more blood loss(P = 0.012),shorter postoperative hospital stay(P = 0.015),and shorter time to intake fluidity food for the first time after surgery(P = 0.015).IL-6,IL-8,TNF-α,CRP measurements as well as CD3+,CD4+ and NK cell count,along with NLR,d NLR,PLR and LMR changed significantly after surgery,and the IL-6concentration was higher at 72 hours postoperatively as well as TNF-α at 24 hours and d NLR,PLR at 72 hours after surgery in the laparoscopic group(P = 0.045,P =0.045,P=0.024,P=0.029).Conclusion: Da Vinci robot-assisted radical colorectal surgery is as safe,feasible,effective as laparoscopic surgery,and is more beneficial for early recovery of patients.Robotic surgery may cause less post trauma stress and less suppression of anti-tumor immunity.
