ObjectivesPapillary thyroid carcinoma(PTC)is the most common subtype of thyroid carcinoma.The widely use of ultrasonography and ultrasound-guided fine needle aspiration(US-FNA)permit the increasing preoperative diagnosis of PTC.Most PTC has a benign behavior and a better prognosis after surgery.The incidence of lymph node metastasis is the most common way of metastasis,which is considered a risk factor for recurrence.Ultrasonography is the most important preoperative imaging procedure in detecting suspicious thyroid nodules and metastatic lymph nodes.However,the sensitivity of ultrasonography in detecting lymph nodes metastasis is relatively low.It is of great necessary to find new methods such as molecular markers to increase the sensitivity of lymph nodes metastasis diagnosis.Long noncoding RNA(lnc RNA)plays an important role in tumor proliferation and metastasis,which was confirmed in human cancers.In the present study,lnc RNA ENST00000489676 has been selected to be associated with lymph nodes metastasis of PTC.The purpose of the present study is to explore the biological mechanism of ENST00000489676 in lymph nodes metastasis of PTC,and also to identi fy the value of ENST00000489676 combined with US-FNA in diagnosing lymph nodes metastasis of PTC.MethodsENST00000489676 has been selected to be associated with PTC lymph nodes metastasis in previous work.The expression levels of ENST00000489676 were tested in PTC US-FNA samples with and without lymph nodes metastasis using real-time PCR.In vitro,the expression of lnc RNA was overexpressed or knocked down,and then the effects on biological abilities such as cell proliferation,migration,invasion ability etc.were observed.We also evaluated the clinicopathologic characteristics and thyroid nodules ultrasonographic features related with lymph node metastasis in PTC.We collected US-FNA samples of PTC and detected the expressions of ENST00000489676.The clinical value of ENST00000489676 as a potential biomarker combined with US-FNA in diagnosis of lymph nodes metastasis of PTC before surgery was evaluated.ResultsIn vitro,the phenotype of ENST00000489676 was confirmed.In gain of function study,when ENST00000489676 expression was overexpressed,the proliferation,migration and invasion ability were all suppressed in PTC cell lines,while those abilities were all promoted in the loss of function study with ENST00000489676 knocked down.The overexpression of ENST00000489676 inhibited the progression of PTC by suppressing mi R-922.ENST00000489676 may play a negative regulating role in PTC growth and progression.Clinicopathologic characteristics of larger tumor size,younger age,microcalcification and multifocal PTC were independent predictors of lymph node metastasis of PTC by analyzing 220 PTC samples.Correlation study indicated that the low expression of ENST00000489676 was independently associated with lymph node metastasis in PTC by detecting the expression level of ENST00000489676 in 220 PTC US-FNA samples.In addition,the sensitivity and accuracy of ENST00000489676 combined with US in the preoperative diagnosis of PTC lymph node metastasis was higher than US alone.ConclusionsThe overexpression of ENST00000489676 inhibited the proliferation,migration and invasion of PTC by suppressing mi R-922.The low expression of ENST00000489676 was associated with lymph node metastasis of PTC.ENST00000489676 may be act as a biological predictor for invasion and metastasis of PTC and may serve as a potential diagnostic and therapeutic target.ENST00000489676 could improve the diagnostic sensitivity and accuracy of US in predicting lymph node metastasis of PTC.
