Price,behavior And Regulation:A Research On Pooled Drug Purchase Policy In China

In order to build a healthy China,it is quite important to construct the mechanism of formulating reasonable medical prices.Under the leadership of the national healthcare security administration,the practice of pooled drug procurement policy in China has developed rapidly since the relevant pilot program,which has far-reaching positive significance for public health and social welfare.Therefore,it has become the focus of theorists and practitioner,and its constraint on drug prices has also attracted much attention.Essentially,the aim of pooled drug procurement is not only to decrease prices,but formulate a mechanism of reasonable medical prices.As a public affair,pooled drug procurement has adopted a new pricing mechanism,which is a kind of group purchase constructed by healthcare security administration.The mechanism has decreased the prices of the chosen drugs obviously.Meanwhile,the prices of generic and brand-name drugs and other drugs with similar efficacy are falling.With the normalization and institutionalization of pooled drug purchase,the drug price system is accelerating to rationalization.However,China’s pooled drug procurement policy has not been able to effectively build a cooperative incentive and restraint mechanism of multiple participants,which has negative influence on the price governance.Therefore,this paper summarized relevant literature research,applied related theories of management,economics and sociology,then conducted a systematic study on this policy according to the theoretical analysis framework of "drug pricing-multiple game-government regulation".Firstly,the policy practice of pooled drug procurement in China was reviewed.Then,based on the process of pooled procurement to form the price of selected drugs and regulate the whole drug market price system,this paper analyzes the mechanism of price governance through the policy.Then,identifying the stakeholders and their multiple demands and main actions in this practice.Also,the paper uses the game theory,starting from the bid game model of bidding drug companies to mixed strategy game model and evolutionary game model of multiple participants,and provides the ideas of preventing collusion and promoting cooperation.Finally,from the perspective of government regulation,we put forward countermeasures and suggestions to guide the coordination of various forces and promote the improvement of pooled drug procurement system in China.The main conclusions of this paper are as follows :(1)the mechanism of drug price formation through China’s pooled drug purchase policy is composed of contract mechanism,competition mechanism and linkage mechanism,which respectively focus on forming reasonable expectation,competitive environment and the reference of price adjustments.(2)Based on the game model of multiple subjects in pooled drug procurement,the paper suggests that the cooperation of multiple agents should be promoted by improving the system design and systematically giving play to the roles of different participants.(3)Referring to the international drug procurement principles,this paper puts forward the basic principles for evaluating the effects of China’s pooled drug procurement policy,and summarizes the current effects and limitations of the policy from six dimensions including economy,desirability,safety,accessibility,fairness and development.Based on the above analysis,this paper proposes that the basic requirements for improving China’s pooled drug procurement system are to combine multiple value,attach importance to information transparency and procedural justice,strengthen the supervision of pharmaceutical enterprises,strengthen the incentive and constraint of medical institutions,and strengthen the guidance of public opinion to patients.As well as,specific countermeasures and suggestions are put forward from the following five aspects: continue to optimize the design of procurement mechanism,promote the integration and linkage of platforms,promote the fine management of hospitals,enhance the supply guarantee ability of pharmaceutical enterprises and improve the structure of pooled procurement system.
