| Background:Stroke is the main negative factor that reduces the quality of life of Chinese residents.It has the characteristics of high incidence,high mortality,high disability rate,and many complications.After stroke somatic symptoms,viscera symptoms and consciousness showed multidimensional characteristics.Rehabilitation training for various functional disorders is the key to improve the rehabilitation efficiency of stroke.At present,the training methods led by the rehabilitation therapists have definite curative effects on motor function,however the cardiopulmonary fitness and the interaction between on consciousness and visceral function have been neglected.Tai Chi is a representative method of active rehabilitation.As its core operation,"Adjustment" can fully mobilize the patient’s attention,memory,cognition and other conscious activities,so as to combine physical exercise with breathing training,and then produce the health rehabilitation effect of dredging qi and blood,nourishing the viscera.Therefore,it is necessary to carry out research on the influence of Tai Chi on the cardiopulmonary fitness of patients in the convalescent stage of stroke,as well as the interaction between consciousness and cardiopulmonary fitness.It is very important to reduce the social medical burden and improve the rehabilitation quality of stroke.Objective:Guided by the "somatic and spiritual harmony" rehabilitation theory,through "Tai Chi six-form" training for stroke patients in the recovery phase,the cardiopulmonary fitness and brain function were collected at baseline and after the training,and then the synergistic relationship between cardiopulmonary fitness and brain function was comprehensively analyzed to explain the somatic and spiritual harmony induced by "Tai Chi six-form".Method:This study is a multicentre,randomized,controlled clinical study,and the sample size was 90 calculated by PASS 15.0.According to the "Diagnostic criteria of cerebrovascular diseases in China(version 2019)",recruit met inclusion and exclusion criteria stroke convalescence patients,based on the random system dynamic area in the middle of the network group randomly,competition into groups,block length is 4 considering areas for hierarchical factors,subjects according to 1:1 were randomly divided into control group and experimental group.Because of exercise therapy could not blind the subjects and coaches,the recruiters,random personnel and evaluators should be blind.On basis of conventional therapy and health education,the control group received walking training,the experimental group received "Tai Chi six-form" training.The training protocol of the two groups were the same:5 times per week,60 minutes per time,for 12 weeks in total,and the training intensity was 55%80%of the maximum heart rate.CEPT and EEG were detected at baseline and at the end of training.Independent sample T test,paired sample T test were used to analyze the effects of "Tai Chi six-form" training on CRF and brain function of stroke convalescences.Pearson and Spearman correlation analysis was used to explore the somatic and spiritual harmony induced by "Tai Chi six-form" training.Results:There was no significant difference in VO2Peak level and O2pluse level between the two groups at baseline(P<0.05).After training,the level of VO2Peak in experimental group was significantly higher than that in control group(P<0.05),level of O2pluse in experimental group was significantly higher than that in control group(P<0.05).There was no significant difference between level of VO2Peak and O2pluse pre and post training(P<0.05)in control group,the levels of VO2Peak and O2pluse in experimental groups were significantly increased pre and post training.Analysis of brain regions related to cognitive function showed that there was no significant difference in alpha and beta band power between baseline and posttraining groups(P>0.05).After training,the absolute value of power of alpha of C3(P<0.05)in experimental group decreased significantly.The absolute value of power of alpha correlation between cognitive function related brain regions in experimental group were significantly positive correlation(P<0.05).Analysis of brain regions related to cognitive function showed that there was no significant difference in alpha and beta band power at the baseline(P>0.05),the absolute power of C3 lead alpha band in experimental group decreased significantly after training(P<0.05).The alpha band of motor sensation-related brain regions was positively correlated(P<0.05).Synergistic analysis of whole brain regions showed that alpha band of cognitive-motor sensation-related brain regions was significantly positively correlated(P<0.05).The correlation analysis between cardiopulmonary function and cognitive function showed that there was no significant correlation between the VO2Peak,O2pluse and the brain areas related to cognitive function in the two groups at baseline(P>0.05).After training,VO2Peak level of experimental groups was significantly correlated with the in alpha band power of Fp1,Fp2,O1 and O2(P<0.05).The O2pluse level after training was significantly correlated with the beta band power of O2,T3,T5 and T6(P>0.05).The correlation analysis of cardiopulmonary function and motor sensory function showed that there was no significant correlation between the VO2Peak,O2pluse and the brain areas related to cognitive function in both groups at baseline(P>0.05).After training,VO2Peak level was significantly correlated with the power of C3,C4,P3,P4 and Pz lead alpha band(P<0.05).After training,O2pluse level of experimental group was significantly correlated with P4 lead beta band power(P<0.05).Conclusion:"Tai Chi six-form" training effectively improve the cardiopulmonary fitness of stroke patients in the recovery phase,and produce a comprehensive,systematic regulation effects on the brain function.There is a certain degree of interaction between the cardiopulmonary fitness and brain function,which has great potential for improving the rehabilitation quality of stroke."Adjustment",as the core operation of "Six forms of Taijiquan",affects the autonomic nerve,which may have a positive regulation on the overall viscera function.This has great potential for improving the quality of stroke rehabilitation. |
