Analysis On The Influencing Factors Of Two - Way Referral In Community Health Service In Qingdao And The Countermeasures

Objective:To know the awarenesss and utilization for community health services and two-way referral of Qingdao between doctors and patients; to survey the views and attitude to a two-way referral between doctors and patients; to explore the influential intention factors for them to participate two-way referral; to find the reasons for referral obstacle and to propose measures for improvement; to provide the oretical and policy basis on the successful implementation of two-way referral.Methods:Through consulting relevant literature and the current relevant policy of national, provincial on two-way referral system, a framework is provided for the countermeasure proposal; through random sampling method to investigate patients and doctors of general hospital and community health service institutions in Qingdao, the awareness perceptions,views and attitudes on two-way referral system of community health services between doctors and patients can be understood; through the semi structured interview statistical methods to analyze the influence factors of both sides involved in the two-way referral system of community health services.Result:The main factors influence the two-way referral system of community health services are the patients’ age, the type of medical insurance and the satisfaction of two-way referral, as well as the medical staff’s work experience, educational level and the cognition of need of two-way referral. At present, the main problems of the two-way referral system of community health services is the medical technology of the community health service institutions level is generally low, human resources and equipment conditions can’t meet the growing medical needs; both patients and doctors especially patients are still lack of systematic understanding and the patients are lack of trust to community health service; the tilt strength of health insurance system is not enough and community two-way referral advantage cannot be well embodied; the governmental investment and policy need to be further improved.Conclusion:The implementation of two-way referral has important significance, for optimizing health resources, promoting the medical institutions functions clear, reducing medical costs, and adapting the aging of population and the change of disease spectrum. The medical institutions, both patients and doctors, government policy and medical security system all can impact the two-way referral, so hospital and the community should make clear duty to achieve an advantage complementary, the roles of medical insurance should be strengthened; the government should play a role in two-way referral system of community health services to perfect the specific operation process design, so as to promote the two-way referral smoothly.
