Objective:To study the genotype and allele frequency distribution of the eNOS (endothelial nitric oxide synthase) gene between the healthy people and IA (Intracranial aneurysm) patients from Qingdao China, based on the clinical material and the analysis of the factors related to the incidence and prognosis of IA.Methods:A total of60cases of IA patient receiving treatment in the department of neurosurgery in Affiliated Hospital of Qingdao University Medical College during the period from January2013to December2013will be chosen as the research subject. All of these patients will receive the imaging examination upon their brains such as CT, CTA and DSA before being operated on. What is more, a group of60healthy volunteers will be chosen as the control group. The peripheral venous blood and genome DNA will be drawn from all people in the two groups above. The PCR will be used to enlarge the target gene fragment whose product will also go through the RFLP analysis and eNOS genotype examination. The statistical analysis will be made upon the result so as to calculate the frequency of G/T allele. The χ2examination will be made to analyze the difference between the frequency of the genotype and allele in two groups and also study the correlation between the eNOS genotype and IA incidence. After the information such as the sex, age, family history, Hunt-Hess grade, IA feature as well as the GOS score gained upon leaving the hospital is collected, a retrospective study will be made on a total of385cases of anterior communicating aneurysms in our hospital during the period from January of1997to June of2012. Based on the incidence condition, preoperative evaluation, intraoperative aneurysm feature as well as postoperative recovery, the analysis on the cause of the aneurysm and the factors related to the prognosis, prevention and treatment will be also made.Results:Among those60cases of IA patients, the ratio of male to female is1to1.14(28to32). The age of the patients ranges from15to76while the average age stands at54.5. What is more,25cases are anterior communicating aneurysms,13cases are middle cerebral artery aneurysm and the rest are IA patients. According to the examination result, the894G/T in the seventh expressed region of the eNOS gene has consisted of three genotypes, namely the GG, GT and TT. For the two alleles (G and T), there are28GG homozygote,14TT homozygote and18heterozygote in the patient group. In the control group, there are39cases,5cases and16cases, respectively. What is more, there exists the statistical difference between the two groups in terms of eNOS-894G/T genotype and allele frequency distribution while the T allele frequency in the patient group is higher than that in the control group. All of385ACoA patients have received the microsurgical operation during which the aneurysm is identified and removed accurately. According to the observation of the extracting position, size, orientation, anatomic relationship with the parent artery as well as the development of anterior communicating artery complex, there are270cases of aneurysm developing at the angle between the left A2and anterior communicating artery,93cases of aneurysm developing at the angle between the right A2and anterior communicating artery and25cases in between. What is more, there are107 cases of aneurysm orienting upward,76cases downward,120cases upper rear,68cases lower rear and17cases orienting in a complicated manner. Besides that, there are340cases of recovery,13cases of improvement,6cases of vegetative state and26cases of death.Conclusion:IA is a disease caused by multiple factors. The multiformity of the984G/T in the eNOS gene might be one of potential contributing factors while the abnormal development of the anterior communicating artery complex and the complicated hemodynamics should be seen as the major cause. The analysis on the cause of the disease as well as the postoperative recovery will be of great reference value to provide new insights into the disease and clinical treatment.
