Objectie:To investigate the efficiency and safety of endoscopic papillary large balloon dilation (EPLBD) after endoscopic sphincterotomy (EST) with different dilating duration for the treatment of common bile duct (CBD) stones.Methods:90cases with CBD stones from April2012to September2013were recruited in the study; all the cases receiving large balloon dilation were randomizedly divided into three groups:0min group,1min group,5min group, with balloon dilating duration for0min,1min,5min separately. The rates of successful stone removal and mechanical lithotripsy, the procedure time and early related complications were compared among the groups.Results:One patient was excluded due to intolerance of the dilation pain; all the other89patients were successful for the stone extraction. Stone clearance rates with single session in three groups were all100%. The rates of mechanical lithotripsy were14.8%(4/27),6.3%(2/32),20%(6/30)in0min group,1min group,5min group respectively (P=0.25). The operation time were0min group (29.5min±8.62min),1min group (24.3min±6.27min),5min group (28.6min±7.58m)(P=0.02). The rate of hyperamylasemia in three groups were0min group14.8%(4/27),1min group0%(0/32),5min group10%(3/30)(P=0.03). No hemorrhage, perforation, cholangitis and acute pancreatitis occurred in any group.Conclusion:Endoscopic papillary large balloon dilation is an efficiency and safety method for the treatment of CBD stones, balloon dilation for1min produced preferable outcomes.
