Objective (1) To introduce a modified post-lateral fusion method with osteotome decompression.(2) To analyse clinical data of patients accepting the operation to demonstrate its feasibility.Methods (1) We analysed clinical data of all the148lumbar spinal stenosis (LSS) patients and the clinical outcomes were evaluated using the JO A and VAS.The fusion rate of facet joint were assessed using sagittal view of CT scan and reconstruction.(2)76cases with spondylolisthesis had accepted our modified operation with posterior lumbar interbody fusion (PLIF).We also use JOA and VAS to evaluate the effect of pre-operative,six months and one year follow-up.The fusion status and loss of correction were judged in CT and plain radiographs.Results (1) All the148patients had got complete follow-up for at least one year. Facet joint fusion rate was98.65%at one year post-operative.There were significat improvements of JOA and VAS score and average decompression time of one segment was8.19±1.05minutes.(2) The average360°fusion time of one segment was98.0±13.4minutes. Post-operative reduction rate of76patients spondylolisthesis was96.8%, with a mean loss rate of1.1%at six months follow-up and0.8%at one year follow-up separately.The fusion rate was76.3%at six month follow-up,98.7%at one year follow-up, and the difference was significant.The JOA and VAS score at the last follow-up was significantly different in contrast to that of pre-ooerative.Conclusion Modified selective decompression with facet joint fusion shows great clinical outcome and fusion rate. thus, it is worth clinical promotion.
