Objective:Patients with gastric mucosal erosion are common in clinic, but there are no specific clinical manifestations. At present, the researches on the pathogenesis and syndrome of TCM theory are not unified. In this research, we have observed and studied the etiology of gastric mucosal erosion, the constitution types of traditional Chinese medicine and Syndrome type of traditional Chinese Medicine. We have investigated the relationship between the etiologies of this disease, TCM syndrome type distributions, the images of gastroscope, gastric pits and TCM Constitution, for the purpose of providing a theoretic foundation for study of the etiologies of this disease and TCM constitution.Methods:1.Case collection:We selected 120 patients who were diagnosed with gastric mucosal erosion in Dongzhimen Hospital’s Gastroscopy Room from November 2014 to February 2015.We collected the gastric images of these patients and recorded the data in the questionnaires.2. Research methods:According to the current relevant researches, results and conclusions, we designed the questionnaire of TCM syndrome of gastric mucosal erosion. By means of question investigation, we collected the relevant information of 120 patients with gastric mucosal erosion.3. We have used the statistical method to analyze the relationship between the etiology of gastric mucosal erosion, the TCM syndrome and TCM constitution.Results:1.120 patients with gastric mucosal erosion were selected into this study. The incidence of the disease in 46~55 years of age are the highest. There is no significant relationship between the incidence of all ages and gender.2. The onset of gastric mucosal erosion has the relationship with emotional factors, living habits and hobbies.Common factors:irritability, eating greasy, raw or cold food, sweets, drinking coffee and tea, non-steroidal drugs, and sit up, etc.3.The distribution of TCM syndromes:cold-heat complex syndrome, stagnated Heat in Liver and Stomach Syndrome, syndrome of liver depression and qi stagnation, syndrome of dampness-heat in the spleen and stomach, spleen-deficiency and qi-stagnation syndrome, qi stagnation and phlegm stagnation syndrome, qi counter flow syndrome, Gallbladderre invading stomach, syndrome of static blood blocking collaterals, spleen-stomach deficiency cold syndrome, Stomach Yin deficiency.The patients of simultaneous occurrence of cold and heat syndrome are more prone to get gastric mucosal atrophy and intestinal metaplasia than the other syndromes.Andthe patients of excess heat syndrome patients are prone to gastric mucosal atypical hyperplasia.4. The endoscopic grade of Gastric mucosal erosion:Grade â…¡> Grade â…¢> Grade â… . Gastric pit type:A type> D type> B type> C type.There are 17.5% patients infected with Helicobacter pylori.5. The distribution of TCM Constitution:gentleness type> yang-deficiency type> qi-depression type> blood-stasis type> qi-deficiency type> yin-deficiency type> phlegm-wet type> wetness-heat type> special inherited type. The patients of yang-deficiency type have more opportunities to get gastric mucosal atrophy, intestinal metaplasia and atypical hyperplasia.6. By means of factor analysis, we get eight kinds of syndrome types:syndrome of liver depression and qi stagnation, Stagnated Heat in Liver and Stomach Syndrome, cold-heat complex syndrome, spleen-stomach deficiency cold syndrome, Gallbladderre invading stomach, qi counter flow syndrome, qi stagnation and phlegm stagnation syndrome, spleen-deficiency and qi-stagnation syndrome.Conclusions:1. There is no significant relationship between the incidence of all ages and gender. The incidence of the disease in 46~55 years of age are the highest. The onset of gastric mucosal erosion has the relationship with emotional factors, living habits and hobbies.2. Gastric pits type, the grading of gastric mucosal erosion, Hp infection in patients with different TCM syndromes in the incidence rate had no significant difference.3. The patients of yang-deficiency type have more opportunities to get gastric mucosal atrophy, intestinal metaplasia and atypical hyperplasia. The patients with cold-heat complex syndrome are more prone to gastric mucosal atrophy and intestinal metaplasia; the patients with excess heat syndrome are more prone to atypical hyperplasia.4.TCM syndrome differentiation of gastric mucosal erosion can be summed up as follows:the basic disease mainly in the spleen and liver; syndrome with excess heat syndrome, cold and heat syndrome, qi stagnation syndrome, Qi deficiency and clip, and phlegm and blood stasis. |
