Effect Of Head Acupuncture On Brain Structure In Stroke Patients With Hemiplegia Based On VBM Technique

Background:The scalp acupuncture has a good clinical effect to treat apoplectic hemiplegia, but the central mechanism has not yet made a comprehensive exposition. In recent years, the acupuncture effect of stroke fMRI study are deeply, but the technology lack of Whole Brain integration mechanism Research, unable to investigate the central mechanism of scalp acupuncture effect to brain repair and damage in microstructure morphology. In this study, we use VBM technology to observe the changes of hemiplegic cerebral gray matter structural damage and repair, explore the central integration mechanism of brain function recovery and scalp therapy central effects sites.Objective:Using VBM technology to research hemiplegia cerebral gray matter damage, repair and scalp intervention effect, and reveal the advantages and central effects sites of scalp treatment for hemiplegia in brain microstructure level. In order to provide the material basis for the clinical efficacy assessment and brain function improvementMethods:A total of 17 hemiplegic patients and another 17 healthy subjects were chosen to research. Us ing a random number table, the patients were divided into two groups,9 cases in scalp group and 8 cases in non-scalp group. Before and after treatment of two weeks, scanning structure MRI imaging data and evaluation of clinical NIHSS and BI scale.Using VBM technology to analyze patients and healthy people’s difference of sports-related brain structure, and the change of two groups before and after treatment with healthy people cerebral gray matter difference. Using another statistical method of generalized linear models to compare the two groups s cerebral gray matter changes before and after treatment.Result:1. Before and after treatment, two groups NIHSS and BI evaluation:Scalp acupuncture before and after treatment NIHSS, BI index scores were significantly different,but non scalp group were no significant difference. Before treatment two groups NIHSS, BI index scores were no significant difference, but after treatment two group NIHSS index scores were significant difference.2.Comparison of hemiplegic patients and healthy people brain gray matter changes: ①Before treatment, hemiplegia patients cerebellum appeared gray matter voxel increase, bilateral cerebral movement, sensation and cognition of cortical gray matter voxel reduce. ②After treatment, scalp acupuncture group’ s right cerebellum,bilateral anterior cingulate, basal nucleus, medial frontal gyrus etal movement related cortical and part of the sensory cortex appears gray matter voxel increase. The cuneus, precuneus, inferior frontal gyrus etal region appear voxel reduce. Compared with 2 weeks ago right cerebellum gray matter growth larger, bilateral anterior cingulate, medial frontal gyrus region of previously reduced appear voxel increase, and there were new voxel growth in the left basal ganglia, parahippocampal gyrus etal. ③After treatment, non-scalp group only has a small range of voxels in a small region growing. Many brain regions appear brain gray matter density widely reduced in the whole brain range,and compared with 2 weeks ago,the range expansion,the number increase. ④After treatment, compared with healthy people, both of two patient groups brain gray matter have new voxel reduce, but the non-scalp group’s brain atrophy number and range were more obvious.3. before and after treatment Scalp group’s cerebral gray matter changes were growing stronger than non- scalp group, and the brain regions mainly in bilateral cerebellar cortex Conclusion:1. Scalp acupuncture has strong clinic effect in treating apoplectic hemiplegia, especially in the improvement of nerve function.2. Changes of hemiplegic cerebral gray matter structure maybe was lead to dysfunction of the material basis of the body. Cerebral and cerebellar cortical gray matter atrophy and growth maybe related in the cerebral movement, sensory and cognitive function decrease and the compensatory mechanism of cerebellar motor coordination ability.3. Scalp acupuncture promote remodeling of brain mechanisms of brain function and the corresponding brain areas related to compensatory mechanisms. The central effects of site maybe May be in the extrapyramidal system and part of sensory cortex. Scalp acupuncture improve nerve function depend on promoting the above cerebral cortex structure remodeling,and inducing compensatory mechanism of brain function.4. With the extension of time,brain gray matter voxel will decrease. Scalp acupuncture group can promote the hemiplegic cerebral structure remodeling. Compared with non-scalp group will better slow the brain atrophy, and promote bilateral cerebellar cortical gray matter remodeling.
