Tinea pedis and manus are the most common clinical presentations of dermatophytosis in China and are also becoming increasingly prevalent across the world because of various factors including changes in lifestyles and incidence of various disease, such as diabates. Tinea pedis and manus infections are caused by dermatophytes. Itchness, soreness, phlyten, and scale symptoms on feet and hands are caused by superficial fungal infections. If this disease were not treated scientifically and promptly, it could lead to some serious disease. Tinea pedis and manus are classified by four types. There are interdigital type, moccasin type, athlete type and hyperatotic type. Hyperkeratotic-type tinea pedis and manus are chronic and recalcitrant to some common antifungal agents like azole. But terbinafine is an antifungal agent from the allylamine class that inhibits the fungal squalene epoxidase enzyme, leading to the intracellular accumulation of squalene, which causes the rapid death of fungi. It has demonstrated activity against most superficial fungal infections, including onychomycosis and dermatomycosis. Vinegar soak of traditional Chinese herbal medicine may have the advantage of antifungal effect.A multicentre, randomized, open, parallel group study was to demonstrate the efficacy of vinegar soak of traditional Chinese herbal medicine versus terbinafine hydrochloride for treatment of hyperkeratotic-type tinea pedis and manus. Terbinafine is an allylamine antifungal agent which has been shown to be fungicidal against dermatophytes of skin. To seek for effective therapy of hyperkeratotic-type tinea pedis and manus, we observed the efficacy, reoccurance and safety of treatment of vinegar soak of traditional Chinese herbal medicine. Participants with hyperkeratotic-type tinea pedis and manus were randomly assigned to two groups. Participants in group Ⅰ were treated with vinegar soak of traditional Chinese herbal medicine externally for 4 weeks, whereas in group Ⅱ, patients are treated with terbinafine hydrochloride externally for 4 weeks. Clinical improvement rates and fungal eradication rates are compared between the two groups at 4 weeks after the initiation of treatment. We entrolled in the project 229 patients which were in Full Analysis Set,202 patients completed 4 weeks of treatment. At baseline, the clinical baseline were balance. It turns out there were significant differences between two groups in clinical improvement rates. The efficacy of vinegar soak of traditional Chinese herbal medicine was 76.47% while the efficacy of terbinafine hydrochloride was 55%. However, there was no significant difference in mycological eradication rates. Consequently, vinegar soak of traditional Chinese herbal medicine externally should be a valuable option for patients with hyperkeratotic-type tinea pedis and manus. 属性不符...
