Clinical Study On Treatment Of Dialysis - Related Hypotension By Shengmai Capsule Combined With Auricular Point Therapy

BackgroundHemodialysis can remove the pathogenic(such as excess water, metabolites, poison, antibodies and immune complex) in the body by the methods of physical,chemical,immune, and supplement necessary electrolyte and bases of the body at the same time,so as to maintain the body water, electrolyte and acid-base balance. Currently, hemodialysis is one of the main measures for the treatment of end-stage renal disease. As the speedy development of the kidney disease diagnosis and treatment level, the renal replacement therapy technology developed very rapidly,the quality of life of the MHD patients with chronic kidney disease (CKD) has received a lot of improvement.But there are still various dialysis complications in clinical, intradialytic hypotension(IDH) is one of common complications.If patients’ blood pressure declined during dialysis,usually need to take measures such as reduce dehydration, discontinue dehydration,or intravenous injection of normal saline solution, to maintain the volume of circulating blood.But these measures may affect the dialysis adequacy, and make the water stores in the body, which will cause the overload capacity, and increase the burden of cardiovascular, which would add further dangers such as heart failure and edema for a long time.Not only affect dialysis effect and the quality of life, even threaten the survival of dialysis patients. So how to maintain blood pressure during hemodialysis and improve the effect of dialysis at the same time,which is widely concerned now.ObjectiveThis research according to traditional Chinese medical theory correlation with the etiology and pathogenesis of intradialytic hypotension, combined with western medicine research, treat IDH qi and Yin deficiency syndrome ESRD patients with the Chinese patent drug Shengmai capsule and Chinese Auricular Therapy. Shengmai capsule supplement qi and nourish yin. It combined the regulating effect of Chinese Auricular Therapy, treat both symptoms and root causes, regulate zang-fu’s functions, thus maintain blood pressure.And explore therapeutic effect of the method of Shengmai capsule with Chinese Auricular Therapy to treat IDH qi and Yin deficiency syndrome patients. MethodThis study selected 40 IDH patients of qi and Yin deficiency syndrome, and equally distributed to treatment group and control group indiscriminately. The treatment group receiving routine blood dialysis treatment, and the control group on the basis of regular hemodialysis treatment to give Shengmai capsule with Chinese Auricular Therapy.Two groups given 8 weeks’treatment as a course of treatment, and monitoring the patients’mean arterial pressure(MAP),the incidence of IDH, unfinished ultrafiltration volume, Kt/V value, and syndromes integrals. Compare the above observation indexes in the change of two groups of patients before and after treatment.Results1 After 8 weeks,the incidence of IDH of the treatment group patients compared with the control group significantly decreased, and the differences between the two groups have statistical significance (P<0.05).2 By comparison, the fluctuation of the MAP of treatment group patients was flatter than the control group, but differences between the two groups were no statistical significance (P> 0.05).3 Unfinished ultrafiltration volume between the two group was no statistically significant difference(P>0.05).4 Kt/V value was no statistically significant difference after treatment respectively compared with 0 and 8 weeks (P>0.05).5 Compareing the change of the syndromes integrals, treatment group and control group patients was no significant difference before treatment (P>0.05); after treatment there was significantly decrease in the syndromes integrals of treatment group (P<0.05); the control group has no significant difference before and after treatment (P>0.05); after treatment the syndromes integrals of the treatment group lower than the control group (P<0.05), the treatment group curative effect is better than the control group.Conclusion1 Shengmai capsule with Chinese Auricular Therapy can reduce the occurrence of low blood pressure, reduce the incidence of hypotension patients with IDH.2 Shengmai capsule with Chinese Auricular Therapy can effectively control patients’ blood pressure fluctuating during dialysis, stable the blood pressure, thus improve the quality of hemodialysis.3 Shengmai capsule with Chinese Auricular Therapy can reduce the clinical symptoms of long-term dialysis IDH patients of Qi and Yin deficiency syndrome,and improve the quality of survival.
