THE STUDY OF THE RELATIONSHIP BETWEEN THE RESPIRATORY DISTRESS IN LATE-PRETERM/TERM INFANTS AND GBSObjective:To explore the relationship of the respiratory distress and GBS infection in late-preterm/term infants within 24 hrs after birth.To enhance the abilty of diagnosing and remeding the early on-set GBS pneumonia.Method:42 Late-preterm infants(34 to 37 weeks)/term infants born in Tongming branch hospital of the 1st affiliated hospital of Xiamen University from January to December of2013 were enrolled into the clinical trial. Neonatal respiratory secretion and blood taken from two groups was cultured and tested for GBS with PCR method respectively. CSF drawn from the same newborn would be cultured and tested for GBS with PCR once any sample of GBS result was positive. The incidence rate of GBS pneumonia in late-preterm/term respiratory distress would be calculated and conclusion the characteristic of early on-set GBS pneumonia.Result:5 cases in 42 late-preterm/term infants developed into the early on-set GBS pneumonia,which accounted for 11.9%. In every 5 airway secretion samples,GBS-PCR is positive and G+bacteria is also found in smear test. The 5 infants with GBS pneumonia had early onset and severe indications, but cured with the treatment of cefotaxim combined vancomycin.Conclusion:There is high incidence rate of early-onset GBS pneumonia in late-preterm/term infants with respiratory distress. Respiratory secretion smear test combined PCR test can diagnose GBS pneumonia quickly. GBS pneumonia progresses quickly, obvious respiratory distress 、high inflammatory index and early pulmonary pathological change is its characteristics. It would be cured with the antibiotic treatment combined supporttherapy.THE STUDY OF THE RELATIONSHIP BETWEEN THE EARLY-ONSET GBS PNEUMONIA IN LATE-PRETERM/TERM INFANTS AND PREGNANT MOTHERSObjective:To analyze the GBS-carrying rate and epidemic character in local pregnant mothers.Method:Late-preterm infants(34 to 37 weeks)/term infants with respiratory distress within 24 hours after birth born in Tongming branch hospital of the 1st affiliated hospital of Xiamen University from January to December of 2013 were enrolled the clinical trial. Their mothers,who received GBS screening before the delivery,were divided into observation and control group according to the result of GBS-PCR screening. To analyze the GBS-carrying epidemic character in pregnant mothers, then to get the high risk factors of neonatal GBS infection caused by vertical transmission.Results:GBS-PCR screening of 15 pregnant mothers are positive. In this group, there are respectively 9 persons whose age is less than 21 yrs and more than 35 yrs, who are illiteracy or junior-middle school educated, who have vaginitis especially irregular theated. In 15 GBS-positive mother, 7 include pregnancy complication, 1 is puerperal fever, 1 is prolonged second stage, 2 are impaired glucose tolerance. There are 1neonatal GBS infection in 6 c/s, and 4 in spontaneous vaginal delivery.Conclusion:Less-educated 、 younger or older mother and irregular theated vaginitis especially colpomycosis mother is the high risk group of GBS infection. There is correlation between pregnant complications and vertical transmission. Vaginal delivery increases the risk of neonatal GBS infection.
