Study On The Relationship Between Body Type And Left Ventricular Diastolic Function In Patients With Essential Hypertension

Purpose:in this study we investigated traditional Chinese medicine(TCM) constitution type and left ventricular diastolic function of hypertension patients in order to guide the prevention and treatment of left ventricular diastolic disfunction caused by hypertension, provide some references to decrease the occurrence and development of hypertension heart failure.Methods : hypertension diagnosis reference the Guidelines for measurement of Hypertension in China(revised in 2011), 9 types were divided according to the Classification and Decision of the Constitution of Traditional Chinese medicine issued by China Association for Traditional Chinese Medicine on April 9, 2009. Generally, 0 to III degree of diastolic function was classified according to the standard recommend by American Society of Echocardiograph, grade 0 represents regular diastolic function and grade 1 and the above level are regarded as diastolic dysfunction in clinical. TCM constitution survey were investigated in 400 patients with hypertension by Questionnaire method, meanwhile, diastolic dysfunction were examined by color-Doppler ultrasound.Results: the TCM constitution of patients with hypertension were displayed in various patterns, Among these, phlegm quality(32.0%),Yang quality(17.5%) and Yin quality 13.5%) were in high propotion. For diastolic dysfunction, phase I, phase II and phase III accounted for the highest, second and minimum proportion reapectively. Differences were also found in classification of diastolic function in patients with different constitution, the proportion of ventricular diastolic disfunction in phlegm quality was higher than other constitution type(P < 0.05); the proportion of ventricular diastolic disfunction in yin-yang harmony, blood stasis and allergic quality were lower than other constitution type(P < 0.05). In patients with qi asthenia constitution, the proportion of class II, III diastolic disfunction was higher than other physical patients(P < 0.05); The object of EF were more than 50% in all patients bring into this study, and lower risk of level III diastolic function decline was found in the patients, meanwhile as into the most of objects were with phlegmy quality, a level III diastolic disfunction were mainly found in patients with phlegm quality.Conclusions: in this study phlegm quality,Yang quality and Yin quality were common constitution in hypertension patients, meanwile diastolic function classification were different in hypertension patients with different TCM constitution. This study shows that TCM constitution and diastolic function classification diagnosis of hypertention patients can give effective guidance to the prevention and control of hypertension and diastolic heart failure. Moerover, TCM constitution and diastolic function classification diagnosis of hypertention patients could play a important role in improvement of patient’s life quality.
