Part I In-tumor Basis and Influential Factors of Pleural Indentation inPeripheral Bronchogenic CarcinomaObjective: To study the in-tumor basis of the formation of pleural indentation (PI) and findthe influential factors correlated with PI in peripheral bronchogenic carcinoma.Methods: Eighty-six cases of peripheranl bronchagenic carcinma that were devided into twognoups:experimental group of 34 cases with PI and cothel group of 52 cases withut PL,weresubjected to general histochemical,immununohistochemical, and ultrastructural study to identify thetypes of fibers and cells in tumorous interstitium. And the relationship was investigated between PIformation and findings of CT and pathology (including in-tumor fibrosis, tumor-chest wall distance,histological type, diameter of tumor,differentiated degree, lymphatic metastases,sex and etc.) bysingle factor analysis and multiple for analysis of Logistic regrssion model.Results:1.The distribution Of in-tumor fibrosis was mainly fibrotic foci in experimentalgroup but diffuse almost over the tumor in control group (P<0.05). Collagenous fibers were higherthan elastic fibers in tumrous interstitium (P<0.01),and they were higher in experimental groupthan in control group(P<0.05).The expressidon of Collagen I was higher than Collagen â…¢(P<0.01),and it was higher in experimental group than in cotrol group(P<0.05).Myofibroblastswere the majority of the interstitial cells in tumor, the amount of myofibroblasts was higher inexperirnental group than in control group(P<0.05).The hyperpltaic degree of collagenous fibers(except for the fegion of hyaline degeneration) was positively relevant to the amount ofmyofibroblasts in experimental group <4=0.849,P<0.01).2The degree of fibrosis was higher inexperimental group than in control group, the proportion of grade 3-4 fibrosis was respectively70.59% and 34.8%(P<0.01).The probability of PI in cases with grade 3-4 fibrosis was 4.533 timesof that in cases with grade 1-2 fibrosis(P<0.01). The tumo-chest wall distance in experimentalgroup was less than in control control group, the proportion of distance less than or equal to 2cm waserspectively 97.06% and 42.30%(P<0.01). The probability of PI in cases with distance less than orequal to 2cm was 17.143 times of that in cases with distance more than 2cm (P<0.01).Statisticaldifference was significan between the histological constitution of two groups,the proportion ofadenocarcinoma and bronchioloalveolar carcinoma was respectively 79.41% and 44.23% inexperimental group and control group (P<0.01).Squamous cell carcinima was the main type incothel group. The probability of PI in cases of adenocarcinma and bronchioloalveolar carcinomawas 5.400 times of that in cases of squamous cell carcinoma (P<0.01).The average diameter ofwtental N was sInaller than ha ofcontfDl group. the Propenion of cases with diameterless or ~ to 3cm was ~vey 6l .76o/ and 38.46mp0.0l).
