The most important checkpoint in cell cycle is a restriction point in late Gl phase. Disorders of cell cycle result in aberrant cell proliferation, gene mutation and tumor aggressiveness.p27Kip1 , a member of cyclin-dependent kinase inhibitors, is a negative cell cycle regulator and a tumor suppressor gene. Mutation of the p27Kipl gene is a very rare event, while aberrant expression of p27Kipl protein is correlated with tumor progression and aggressiveness. Hepatocellular carcinoma(HCC)is one of the common and highly malignant neoplasms with high recurrence rate and poor prognosis, especially in China. We therefore investigated the relationship between p27Kipl protein expression and apoptosis to elucidate clinical significance and prognostic value in HCC.Material and MethodWe measured p27Kipl protein expression level in 33 HCC specimens, 25 casesof adjacent noncancerous tissues and 11 cases of normal controls by SPimmunohistochemical method , analyzed the correlations between the p27 Kipl expression and clinicopathologic factors such as age,gender,cirrhosis , HBsAg , AFP , tumor size , metastasis , differentiation , Edmowon stage , PCNA and apoptosis in HCC. TUNEL immunohistochemical method was used to measured the rate of apoptosis. The clinico-pathologic data are complete. No patients had received chemotherapy or radiation therapy before sugery.Histologic grading was performed on hemotoxylin and eosin(H&E)-stained sections.The labeling index of p27Kipl (p27LI)was determined in those tumor areas with positively staining nuclei. The average percentages of positivity were recorded as the p27LI for each case. The apoptic index(AI)was expressed as the ratio of positively staining tumor cells and apoptic bodies to all tumor cells,given as a percentage for each case.Result and discussionThe average p27LI in HCC was 61.3 ?23.6%, significantly higher than that in adjacent noncancerous tissues ( p27LI=44.5 ?4.6%, 0.01
