Insulin resistance exists to varying degrees in type 2 diabetic patients. Hyperinsulinemia as a main characteristic of insulin resistance is one of the important research topics at present. The change of insulin level in type 2 diabetic patients may have certain effect on plasma endothelin. Great members of studies have been done on the changes of plasma endothelin density in type 2 diabetic patients with complications. It is reported by documents from abroad that there is a relationship between increased plasma endothelin and type 2 diabetic patients with blood vessel complication. Vascular endotheliocyte injury in type 2 diabetic patients may be an important cause of angiopathy, and endothelin is a vasoconstrictive peptide secreted by vascular endotheliocyte. It is reported that animal experiment in vitro finds high dense insulin has an enhanced sensitive action on plasma endothelin. There still are different opinions on the changes of plasma endothelin in diabetic patients; there aiso are different reports of blood glucose抯 effect on plasma endothelin. At present, many studies concentrate on the changes of plasma endothelin level in diabetic patients with complications and little on the relationship between hyperinsulinemia and plasma endothelin in first diagnosed type 2 diabetic patients. In this study oral glucose tolerance test (OGTT) and insulin release test were done and fasting plasma endothelin was measured in eighty first diagnosed type 2 diabetic patients. The patients were divided into two groups: high insulin group which two-hour postprandial insulin~4OmIU/L relatively high insulin group which two-hour postprandial insulin<4OmIU/L,then compared them with normal group, analysed the relationship between plasma endothelin and insulin in type 2 diabetic patients with different insulin levels. 2 Results showed: I .The plasma eridothelin level of type 2 diabetic patients was significantly higher than that of normal group. 2. In type 2 diabetic patients, the plasma endothelin level of high insulin group was significantly higher than that of relatively high insulin group. 3. Fasting plasma endothelin was positively correlated with fasting insulin and with blood glucose. Conclusions: 1. High insulin level may be an independent injurious factor in first diagnosed (patients) diabetes mellitus. 2. Increased plasma endothelin in type 2 diabetic patients remind us watching out for early stage blood vessel complication and guide clinical drug application.
