Liang weixiongApoplexy is a frequent cerebrovascular disease in China, in which ischemic stroke is about 60?0% of apoplexy. In the recent years, combined treatment of apoplexy has been regarded extensively and the number of related studies is growing constantly. On the ground of reviewing the studies on combined treatment of ischemic stroke, 609 cases of ischemic stroke were analyzed retrospectively under the guidance of "Criterion of diagnosis and treatment of ischemic stroke"("Criterion" and patients with "Feng-tanyuxue-bizumailuo syndrome"in acute ischemic stroke were selected for the present study, the combined treatment of Traditional Chinese Medicine(TCM) was performed on these patients and clinical efficacy was evaluated by using randomized controlled study method.1.Reviews of the TCM theory of pathogeny and pathogenesis of ischemic stroke and the clinical application of combined treatment.The pathogenesis of TCM of apoplexy comprises vacuity, fire, wind, phlegm, qi and blood, "Feng-tan-yuxue-bizumailuo syndrome"is a frequent syndrome in ischemic stroke.It's considered that treatment with integrated Chinese traditional and western medicine, multiple routes of drug administeration and application of multiple technological methods is one of leading trends in apoplexy management after reviewing therapy of TCM in ischemic stroke. Combined treatment methods include combined treatment of TCM, combined treatment of western medicine and combined treatment of integrated Chinese traditional and western medicine. Anyway, there exists some shortcomes andunsolved problems in combined treatment.2.Retrospective analysis of 609 in-patients of ischemic stroke and the combined treatment of TCM609?patients discharged from Jan 1st, 1995 to Dec 31 th, 1999 which had been diagnosed and treated at the direction of 揅riterion?were analyzed retrospectively. 460 patients which had received the nerve & muscle therapy, or intravascular laser irradiation on blood (ILIB) ,or brain-reflection therapy, or acupuncture therapy except for basic treatment were assigned to combined treatment group, the another 149 patients which only received basic therapy were assigned to control group. There were no significant differences between the two groups in sex, age, hospitalization, past history, personal history, inducement, location of the lesion, the number of focus, proportion of 1CM syndrome type ,etc.The results showed that of 609 patients, markedly effective rate was 52.5% (320/609) and effective rate was 96.4% (576/609). The total effective rate of combined treatment group and control group were 96.7% and 88.0%, respectively. The clinical efficacy of combined treatment group was superior to that of control group (P<0.05) and in acute stage, the clinical efficacy of the prior was also superior to that of the latter. It suggested combined treatment of 1CM improves the clinical efficacy in management of ischernic stroke. Data also showed 250 cases with 揊eng-tan-yuxue-bizu-mailuo syndrome?was 40.89% of all cases and 199 cases were in acute stage. The results suggested that 揊eng-tan-yuxue-bizumailuo?syndrome is a frequent syndrome in ischemic stroke.3.Randomized controlled trial of the combined treatment of 1CM on 75 cases of acute ischemic stroke75 patients of acute ischemic stroke with 揊eng-tari-yuxue-bizumailuo syndrome?were randomly divided into combined treatment group (n~53)and control group (n=22) at the ratio of 2:1. Under the guidance of the 揅riterion? patients of the two groups were treated with specific patent Chinese medicine targeting the corresponding syndrome. Nerve & muscle therapy, intravascular laser irradiation on blood (ILIB) and rehabilitating training were used for patients of the combined treatment group except for the above therapy.
