ET1OLOGICAL FACTORS AND PBMC-SECRETED Thl/Th2 CYTOKINES IN THE PATIENTS WITH DERMATITIS AND ECZEMA

Dermatitis and eczema are common skin diseases.Their etiological factors remain complex and uncertain, but mostly concerned with allergic reaction. At present, it has been considered that the outside contact factor is the usual reason. As one of the method of detecting allergen of dermatitis and eczema, patch test is sensitive and peculiar. It has been used widely in clinic practice and gained extensive attention of dermatologist and allergic reaction workers from many countries. So it is important to study the etiological factors for prevention of dermatitis and eczema in clinic practice. The neomycin is the usual topical antibiotic. The tendency of its allergic reaction is increasing. In our study, patch test was performed using standard antigen of "bian min" and gentamycin antigen in 337 dermatitis and eczema, while 50 healthy without dermatitis are the control. It was discovered that the positive reaction rate of contact dermatitis is 58.09%. The most frequent allergens are nickel sulfate (25/136), fragrance mix (16/136), Ursol D (14/136). The positive reaction rate of eczema group is 42.29%. The most frequent allergens are cobalt chloride (33/201), nickel sulfate (29/201), potassium-5-dischromate (24/201). The positive reaction rate of control group is 6%. There were 9 cases being the positive reaction to neomycin in dermatitis and eczema groups, 4 cases of gentamycin, 3 cases of kanamycin,! case of streptomycin.In our study, we find that there was notable different between the dermatitis group and control group hi patch test positive rate. It showed that the sensitization to allergen of the dermatitis and eczema patient is higher than normal persons who are insensitive to patch test reagents. And it also showed that there is crossing-sensitivity between neomycin and other aminoglycoside antibiotics. This result is very important for clinical doctors of dermatology using medication to treat dermatitis and eczema. In this experiment,over half of contact dermatitis patients can be found their allergens, and about half of eczema patient also can be checked out their shadiness allergens. It is important clinical value of patch test to investigation of etiological factor of skin disease such as dermatitis and eczema.The contact allergy is classic delayed-type hypersensitivity mediated by cellular immunity react to contact allergen. Today's most study shows that the dynamic equilibrium of Thl and Th2 play an important role hi the development of contact allergy. The immune model of Thl and Th2 is closely related to cytokine level in it. The cytokine composes a network system, when the net is out of balance, it will make for disease. So attention has been drawn to cytokine secretion disorder caused by the unbalance of Th subgroup's function hi contact allergy. We separate the PBMC from peripheral blood of contact dermatitis and eczama patients, and normal control use routine by Ficoll-Hypaque. We adjust the PRMI-1640 contain 10% calf serum to IXlOVml, then add ConA 0.05mg/ml, cultured in 37癈 5%CO2 incubate box, collect serum and save hi -70 癈. The cytokine hi serum are detected use BA- ELISA. The data are treated using t-test. From the experiment, we find the IFN-gema level of dermatitis and eczema patient is higher than the control. But the IL-4 level is lower. There is statistics distinguish between two groups (pO.Ol). It shows that it's the predominance of Thl result hi contact allergy. The IL-12, being the determinate factor of Thl differentiation, affect the Thl/Th2 balance in contact-6-allergy and change immune reaction characteristic. In our study, the IL-12 level of dermatitis and eczema group is notable higher than the control (P<0.05). So IL-12 promote the development of contact allergy. There is no notable difference between the dermatitis group and control group of IL-10. This result differ from the past research for IL-10 level in different period of disease.
