| IntroductionWith the development of the modem surgery and the knowledge of the disease, many difficult operations could be done now. When the hard operations were done, the operative time prolonged and the patients endure the server hurt. The healing time comes longer and the chances of infection become big. If the infections can't be control, the patients maybe die and the surgical therapy was foiled at last. Enterococci is a common Nosocomial: pathogen. Date collected in the United States by the National Nosocomial Surveillance System (NNIS) indicated mat Enterococcus was the second most commonly reported Nosocomial pathogen.Enterococcus faecium(E.faecium) is one of the enterococci. It is one of the normal intestines bacterial of the human and animal. It could cause infection in some condition and lead to bad sequent. Enterococci are easy to drug-resistant. Vancomycin-resistant enterococci are high drug-resistant. Vancomycin-resistant enterococcus faecium(VREF) is the mostly VRE. It is almost all antibiotics-resistant . It is worse mat the experiments in vitro had proved the drug-resistant gene of VRE could easily transmit to the Staphylococcus aureus and generate an almost all antibiotics-resistant bacteria. So our country and other countries are all interesting in the research of prevent and therapy VREF infection. If the infection happened, they all choose the sensitive antibiotics from the drug-sensitive test in vitro. It is lucky that we didn't find many VREF in the past 10 years all over the word. But prevent the VREF infection is very important because we had made a mistake in treating the MRSA. We try to discuss the risk factors of Enterococcus faecium infection after surgery and how to prevent it, then to therapy it from the study to the mechanism of drug-resistant and the reason of infection.Objective1. To discuss the risk factors of Enterococcus faecium after surgery.2. To study the clinical therapy ways of the antibiotics-resistant Enterococcus faecium after surgery.Materials and Methods1. Origin of the samplesThe experiment samples were from the patients of our surgical department Including blood, urine, sputum, exudation of wound, flow of the abdomen cavity , choler. exudation of CVP duct and so on.2. Materials and InstrumentThe antibiotics of drug-sensitive test were bought from National drug and bio-product examining institute. Vancomycin was bought from the great wall pharmaceutical factory of Zhangjiakou. Gentamycin was brought from Sanming pharmaceutical factory of Fujian. M-H culture medium was bought from the Zhejiang Military quarantine. The bacterial identified cards were GEP-TA and API, which bought from Bio-Merieux company of France. Vitek-AMS automatic microorganisms identified instrument was bought from the same company.3. The information of hard-treated E.faecium infected patientsThere are 5 cases patients who had little effected done by antibiotics.4. Experiment methodsUse the Vitek-AMS automatic microorganisms identified instrument to identified the bacteria. Choose the antibiotics based on National committee for clinical laboratory standards. We used K-B paper diffuses. Culture the bacteria for 24h in the M-H plate with 5% sheep RBC. Then put the antibiotic paper in and measure the MIC. Transform the MIC to sensitive(S), resistant(R) or middle(M). The target of infection are temperature of the patients(T), White blood cell(WBC) and the N(%). The feeling of patients are me secondary target The kind of bacteria is proved by culture. Hard-treated patients are the people who had little effect done by the antibiotics but the antibiotics are sensitive to the E.faecium in vivro.5. Isolation methodsWe isolated the patients as soon as the E.faecium infection was found.
