| OBJECTIVE: This single-institution study aims to find out the states of diagnosis and treatment of patients with pancreatic cancer in the first affiliated hospital of medical school of Zhejiang university, and to discuss the outcomes of patients with pancreatic cancer undergone different surgical approaches, and to conclude diagnosis and treatment experience of pancreatic cancer.METHODS: We collected 321 records of patients with pancreatic cancer at our hospital between January 1990 and December 2000, and established a sound database with Access. All patients were followed-up. All data were analyzed using statistic software package SPSS 10.0. Both univariate and multivariate models were applied for survival analysis. Kaplan-Meier method is used mainly, and log-rank test are performed meanwhile.RESULTS: The radical resection rate of 17 patients early diagnosed (stage I and II ) is 100%. 73.33%, 40.00% and 30.00% patients survived for l,3,5years respectively. Abdominalpain and swell are the most common symptoms of patients with cancer of the head, body or tail of the pancreatic. The resection rate is 37.70% (95/252) and the radical resection rate is 23.81%. The operation period is associated with outcome. Survival of the radical resection group were 55.77%, 17.40% at 1, 3 year and 13.92% at 5 years, and compared to other groups, there are statistic significance (p<0.01), whereas, there are no statistic significance between the others (p>0.05). CONCLUSIONS:1. The improvement of early diagnosis rate of pancreatic cancer can increase the survival rate obviously.2. Base on the situation of our country, we think all patients aged above 40 with abdominal pain and swell should be examined by ultrasound, and patients who was suspicious of pancreatic cancer should be examined by CT scan routinely.3. The team special for pancreatic cancer treatment and the standardization of operation are important for improvements the outcomes.4. Radical resection is the only effective procedure. The palliative treatment can't prolong the survival of patients.5. The diagnosis and treatments of pancreatic cancer of our hospital keep ahead in the domestic. |
