[Objective] To explore the diagnostic values and clinical significance of detecting P-selectin and interleukin-lalpha of pleural effusions in differentiating tuberculous and malignant pleural effusions. [Method] Applying enzyme-linked immunosorbent assay(ELISA)method to detect the content of P-selectin and interleukin-lalpha in centrifuged supernatant of 48 tuberculous pleural effusions and 50 malignant pleural effusions and in cultured supernatant of carcinoma cells which were separated from 22 malignant pleural effusions and cultured for 72 hours . [Results] The mean content of P-selectin of malignant pleural effusions (18.7+8.4 U g/L ) was markedly higher than that of tuberculous pleural effusions (7.4+3.3 U g/L),the difference between them was very notable(p<0.01).If 10 U g/L was regarded as limit,the sensitivity,the specificity and the rate of right of diagnosing malignant pleural effusions were 84%, 85.4% and 84.7%,respectively when the content of P-selectin of pleural effusions was higher than 10 U g/L . The mean content of BL-1 a of malignant pleural effusions (76.8+ 24.9ng/L) was elevatedly higher than that of tuberculous pleural effusions (36.4+18.7ng/L),the difference between them was notable 0.05).If 50ng/L was regarded as limit,the sensitivity,the specificity and the rate of right of diagnosing malignant pleural effusions were 64%, 72.9% and 68.3%respectively when the content of IL-1 a of pleural effusions was higher than 50ng/L . The content of P-selectin and IL-1 of malignant pleural effusions was positively correlated (r=0.6872 ) . The mean content of P-selectin and IL-1 a of cultured supernatant ofcarcinoma cells was 30.7+5.8ng/L and 20.4+6.4pg/L, respectively. [Conclusions] P-selectin and IL-1 a can be regarded as effective laboratory marks of differentiating tuberculous and malignant pleural effusions. The content of P-selectin and IL-1 a of malignant pleural effusions was higher than that of tuberculous pleural effusions. P-selectin and IL-1 a were positively correlated in malignant pleural effusions. Some carcinoma cells can express P-selectin , and some carcinoma cells can produce IL-1 a .P-selectin and IL-1 a perhaps colaborate to form malignant pleural effusions and to promote carcinoma cells to disseminatedly metastasize to pleura.Postgraduate:ZHAO YunFeng(Respiratory speciality)Directed by WANG YingNian...
