Transurethral resection was begin from 1930. Having less injury and lower mortality than traditional operation. We began to use the technology in China from 1980. At present, transurethral resection of the prostate(TURP) and transurethral vaporization of the prostate (TUVP) are prevalent for benign hyperplasia on clinic. And TURP had been a "gold standard" operation for benign hyperplasia. But TURP have some complications including erectile impotence and anterograde ejaculation, and so on. Now, There are some controversy about erectile impotence following TURP.The topic study erectile function following TURP via clinical data and experiment. Follow-up for patients accepted TURP. According to age,resected tissue of prostate,preoperative sexual function and postoperative hyponatrema ,we compared alternation of postoperative sexual function,on effect of above mentioned factors,respectively. Canines received antegrade resection of the prostate. We study the effect of erectile nerve following TURP.The main objects of this study: (1) Record complete clinical data of TURP cases; (2) Obtain follow-up data of TURP;(3) Analyze factors of affecting erectile functionpost-TURP;(4) Establish animal model of the operation;(5) Observe the degree of injury after operation by microscope;(6) Complete NO activity assay of Corpora Cavernosa of penis;(7) Analyze the effect of erectile nerve post-operation.Materials and methods257 cases of benign hyperplasia who accepted TURP from July 1996 to July 2001 were involved in our study. According to age (>or≤60) .resected tissue of prostate ( > or ≤20g) ,preoperative sexual function and postoperative hyponatrema ,we compared alternation of postoperative sexual function,on effect of above mentioned factors,respectively. Establish animal model of the operation . Observe the degree of injury after operation by microscope Complete NO activity assay of Corpora Cavernosa of penis before pre-operation and post-operation. Analyze the effect of erectile nerve post-operation. All data were analyzed by SPSS 10.0 Statistic soft-shell.Result1 Erectile function with older than 60 years has high impotence risk than that younger than 60 years following TURP.2 Erectile function have no significant change between beyond 20 gram of resected tissue of prostate than lower 20 gram of that3 Erectile function with preoperative partial erection has high impotence risk than fully potent.4 Post-operative hyponatrema has no significant relation with alternation of erectile function.5 TURP in the canine model reveals only a shallow remnant of necrosis at the site of resection with less than 1.6mm deep of necrosis6 NO activity of Corpora Cavernosa of penis has no significant between preoperation and post-operation.ConclusionAge and erectile function of preoperation have significant effect for erectile function of post-TURP. Resected tissue of prostate and post-operative hyponatrema have no relation with alternation of post-operative erectile function in statistics. TURP in the canine model reveals only a shallow remnant of necrosis at the site of resection with less than 1.6mm deep of necrosi. No histopathologic change on surrounding tissue. Erectile nerve has no injury after operation. We think TURP don't result in post-operative erectile dysfunction if the operater do it regularity.
