Postpartum hemorrhage is the primary cause that leads to the death of parturient in our country nowadays, Its incidence rate is from 2%-3% of all woman in labor, Among which, 70%-80% is due to uterine atony. Uterine contraction is controlled by neural, endocrine, mechanical, mETRAblic factors, In which, Endocrine factor is paid more attention. Many kinds of hormone are involved in the regulation of uterine action by autocrine or paracrine fashion, Such as oxytocin, prostaglandins and endothelin et al. endothelin (ET) is an important substance that modulates celluar growth and hormonal mETRAbolism and widely exists in tissues and cells. It has been known dial ET plays an important role in the growth of endometria and the contractivity of myometrium. ET acts only when binding with its peculiar receptor on the surface of target cells. This experiment examine the value of ET receptor A subtype ( ETRA ) messenger RNA ( mRNA ) applying the method of reverse transcriptase polymerase chain reaction ( RT - PCR) and also determine the quantity of hemorrhage in two hours after labor in order to explore the relationship between the level of ETRA gene expression and the volume postpartum hemorrhage.Materials and methodsSelect 60 pregnant woman who deliveried by cesarean section in our hospital, They are devided into two group according to the state of parturiency, one group is pre - parturient and the other is post - parturient. Each group is 30 women. The criterion is followed: (1) The pre - parturient group, FETRA1 monitoring indicates no regular uterine contraction is found and cervical apertura is not open. (2) The post -parturient group, According to the standard of textbook (gynecology and obstetrics)No. 5 edition. Both group are primiparous and term pregnant. Before operation, None of women has pregnancy-induced hypertension syndrome or complication of medical and surgery, None takes relaxant drugs, prostaglandins derivative and oxytocin, The function of liver and kidney of all woman are normal. After removal of samples, Myometrium is injected 10u oxytocin immediately, and one methyl carprosl suppository is given via the rectum after coming back to the ward. The gravidas of two group have no significant difference in maternal age, gestational week, times of delivery, maternal throm-bcyte and circulatory concentration of calcium. Fresh myometrial samples from each woman were exercised immediately after delivery of child when not used uterine contractile agents. A stretch of smooth mucle about 0. 5 -1.0 gram was cut from the upper, middle edge of the transverse incision abacterially, After dealed with by sodium phosphate buffer , the biopsy were frozen in liquid nitrogen at once; and later were stored in a 70 C refrigerator when transferred into lab. The quantity of postpartum hemorrhage of every gravida in 2 hours after labor is determined making use of the methods as follow ramniotic fluidhematocrit detection, weight andvolume calculation. The value of ETRA mRNA on myometrium is examined by the means of RT - PCR, RT - PCR kit was bought from TaKaRa corporation, ETRA primer, internal standard GAPDH primer were synthesized by TaKaRa company. All documents were managed by 10.0 SPSS stastistics ware , applied T test and stainght line correlation analysis.ResultThe myometrium of both group expressed ETRAmRNA, The level of ETRA mRNA expression in myometrium of the post - parturient group is 0. 8259+0. 04827, While the level of ETRAmRNA expression in the pre - parturient group is 0.6867+0.05963, After comparison , The level of ETRA mRNA in the post - parturient group is elevated remarkably than that in the pre - parturient group (p<0.01). The quantity of postpartum hemerrhage in the post-parturient group (40 0. 00+104. 36ml) is significantly lower than that in the pre -parturient group (575. 40+112. 31ml) p < 0. 01.
