Objective To study the effect of autogenous oxygenator cardiopulmonary bypass(CPB) and artificial oxygenator CPB on lung.METHOD 16 adult mongrels (weight 15 ?1kg) were randomly divided into two groups, 8 for Experimental group, in that the dog's own lung was used as oxygenator; 8 for Control group in that the artificial lung(bubble oxygenator) was used as oxygenator. In Experimental group, The CPB system was established by aorta and left atrium cannulation (for the left-heart bypass), pulmonary artery and right atrium cannulation (for the right-heart bypass). In Control group the CPB system was established by right atrium and aorta cannulation. The technique of myocardium protection and cardiac arrest were same in both groups.( Cold crystalline cardioplegia) CPB was controlled running for 1 hour in both groups. Theamount of leukocytes (samples of right atrium and left atrium ), the pulmonary artery pressure, the oxygen pressure were measured at the time point of before-CPB and the 5th 30th 60th 90th minutes after CPB in both groups. And the ratio of leukocyte was calculated. Lung biopsies were obtained before and after CPB in both groups for microscope pathological examination. The oxygen pressure were measured at the time point of the CPB beginning and 15th 30th 45th 60th minute during CPB in both groups.The pulmonary artery pressure was monitored continuously during CPB in Experimental group.Rethults Autogenous oxygenator CPB technique can provide satisfactory oxygenation during CPB. The ratio of leukocyte after CPB was significantly lower in the experimental group than that in the control group at different time point (p < 0.02). The pulmonary pressure after CPB was significantly lower in the Experimental group than that in the control group at the 30th 60th 90th time point(p < 0.02).The oxygen pressure after CPB was significantly higher in the experimental group than that in the control group at different time point( p < 0.02). Lung tissue histologic examination revealed that in Control group there were more obvious intraalveolar hemorrhage leukocyte adhesion and interstitial edema compared with Experimental group.Conclusion The autogenous oxygenator CPB technique can providesatisfactory oxygenation and get better lung protection effect.
