| Objective: To observe the changes of the tumour necrosis factor-alpha (TNF- a ) in the plasma and the expression of vascular endothelial growth factor (VEGF) in the synovial of the collagen II induced arthritis (CIA ) in rats, to explore the role of TNF- a and VEGF in rheumatoid arthritis. To observe the influence of the bizhongxiao(BZX) decoction on TNF-a in the plasma and the expression of VEGF in the synovial of C II-induced arthritis (CIA) in rats. To explore the mechanism of the BZX decoction in the treatment of rheumatoid arthritis disease.Methods: Sprague-dawlay rats (n-75) were randly divided into four groups, 1Normal control group (Normal group, n=5); 2Model control group (Model group, n=16); 3Methotrexate treated group(MTX group, n=25); Bizhongxiao decoction treated group (BZXD group, n=29) .The rat experiment arthritis model was established by subcutaneous injection of II collogen and complete Freund's adjuvant to the Model group, the MTX group and the BZXD group, but the Nomal group was not injected.Rats of the BZXD group and the MTX group were respectively treated with BZXD and MTX, and the BZXD groupwas treated twice everyday, and the MTX group was treated once every week, and the model group were given distilled water, and the normal group drank freely.The blood of the normal group was gotten by breaking their heads in the 25th day, and that of the model group, the BZXD group and the MTX group was gotten by the same way in the 25th, 30th, 35th, 40th, 45th day.The plasma TNF- a content of the four groups were tested with the radioimmunologic assay in different times.The pathlogic changes of synovial was observed with HE method staining and the expression of VEGF was observed with immunohistochemistry by light microscope, and the expression of VEGF was analyzied with image processing system.The symptom of arthritis was evaluated by the method of arthritis index integra.Results: (1) General condition observtions: The diet of the model group rats reduced, and the mean weight of the model group was lighter than that of the BZXD group and the MTX group.There were some symptoms in the model group , for example, dry and dall hair, but there were not these sympoms in the normal group, the BZXD group and the MTX group. (2) Sixty-two rats occurenced arthritis in the seventy rats , and the incidence of arthritis was approximately 88.57%..A part of rats occurenced arthritis after they were immuned with II collogen in the seveth day, and three group model rats were immuned with II collogen the second in the 21st day, then arthritis ofthree group model rats got gradually worse In the 30th day,arthritis of the BZXD group and the MTX group relieved gradually, but that of the model group got enven worse. (3) Result of the evaluating integra of arthritis index: The integra of arthritis index of the model group, BZXD group and MTX group was increasing with the time elongation,but there was no significantly diffirence on integral of arthritis index among by comparison 14 days ago The integral of arthritis index.arthritis of the model group was higher than that of the BZXD group 18 days later(p<0.01 or p<0.05) and was higher than that of the MTX group 21 days later(0.05).The integral of arthritis index of the BZXD group and the MTX group was the highest in the 21st and the 25th, but was lower than that of the model group.The integral arthritis index of the BZXD group and the MTX group began to descent in the 30th day, but the model group still rose. (4) Observation of the pathomorphology: Normal synvial tissues were observed in the synovials of the normal group, but many nevasculars and thickening synovials were observed in the model group, and a few of nevasculars, inflammatory cells and mild thickening synovials were observed in the BZXD group and the MTX group.The nevasculars of the model group were notably more than those of the BZXD group and the MTX group (P<0.01),but there were no deffirence between the BZXD group and the MTX group(p>0.05).
