Objective: To evaluate the effect of combined splenorenal shunt and portaazygous devascularization for treatment of portal hypertension. Methods: 7 cases of portal hypertension undergone the combined devascularization procedure between 20002002. The main surgical procedures were splenectomy, portaazygous devascularization and splenorenal shunt. The follow up points included hepatic fundation, blood routine test and FPP in operation, which were analyzed prospectively, compared with 8 cases performed devascularization alone. Results: The clinical effect was satisfied, 1 case appeared as hepatoencephalophathy postoperative hepatic. Before operation free portal pressure (FPP) maintained at the level of 29.43 + 4.86 cmH20,after operation FPP maintained at the level of 23.63 ?5.04cmH2O. The FPP of 8 cases performed devascularization alone maintained at the level of 30.87?.55 cmFbO before operation, after operation it was 23.50 ?.55 cmH2O. ConcIusions:The combined procedure could be considered as a rational and practical procedure for treatment of patients with portal hypertension. If FPP was more than 25 cmFH2O, we thought the operation of Combined splenorenal shunt and portaaygous devascularization should be performed. Further clinical effects for this procedure need more cases and longer term follow-up.
