IinstructionEarly detecting minimal tumor cells dissemination in the blood, lymph, marrow, abdominal cavity and the other tissues or organs could give us a valuable warrant to predict the recurrence and prognosis. The method of RT-PCR used to detect tumor specific and tissue specific genes is so sensitive and convenient. It can effectively detect the minimal tumor cells in the blood, marrow, abdominal cavity and peripheral blood of gastric cancer. Our experiment detected both tumor specific gene CEA and tissue specific gene CK20 expression in the gastric region venous blood of gastric cancer patients, and evaluated the clinical significance of minimal cancer cells in blood of gastric cancer patients by RT-PCR and the relationship between CK20 and CEA mRNA expression in gastric region venous blood and the pathobi-ological characteristics of gastric cancer.Material and MethodBlood samples of gastric region venous blood were obtained from 36 gastric cancer patients attending the department of oncology at the first hospital of China Medical University. We randomly selected 10 tissue samples of gastric cancer and 2 blood samples of gastric cancerwith liver metastasis as positive controls, 10 blood samples of gastric region venous blood from gastric benign lesion as negative controls.10 ml blood from right gastroomental vein was collected into a tube containing heparin as soon as we opened the abdominal cavity. The fraction of mononuclear cells was isolated by density centrifuga-tion. Total RNA was extracted using TRIZOL reagent. Then RT-PCR was performed for CK20 and CEA mRNA. At last all RT-PCR products were analysed by 2% agarose gel electrophoresis and visualised on a UV transilluminator after staining with ethidium bromide. The association between the CK20 and CEA mRNA expression and the pathobiological characteristics was examined by X test.ResultsCOO and CEA mRNA expressions are positive in both 10 gastric cancer tissue samples and 2 gastric region venous blood samples of gastric cancer patients with liver metastasis. They are all negative in 10 gastric benign lesion patients. In the 36 patients with gastric cancer, 28(77. 8% ) were CK20 mRNA positive expression, 21(58.3% ) were CEA mRNA positive expression, 13(36. 1% ) were both CK20 and CEA mRNA positive expression, 32(88. 9% ) were either CK20 or CEA mRNA positive expression. CK20 and CEA mRNA expression in the gastric region venous blood samples has a significant difference between gastric cancer and gastric benign lesion( P < 0.01).CK20 mRNA positive rate was 58.3% (7/12) at a size <5cm, 70.8% (17/24) at a size 255cm; 100% (2/2) at localized type, 65.5% (19/29) at invasive type; 60% (6/10) at good differentiation, 69.2% (18/26) at bad differentiation; 70% (21/30) at inva-sive depth of m-ss, 50% (3/6) at invasive depth of se; 80% (4/5) at massive growth type, 64.5% (20/31) at nest and diffuse growth type; the group of lymph node metastasis.- 53. 3% (8/15) at N1, 73.7% (12/19) at N2; the number of lymph node metastasis: 68. 2% (15/22) at 6, 58.3% (7/12) at >6; TNM staging: 73.7% (14/ 19) at I + E stage, 53.3% (8/15) at I + IV stage. There was a increasing tendency of CK20 mRNA positive expression of gastric region venous blood at the size 5cm, localized type, bad differentiation, massive growth, and lymph node metastasis N2. but there was no significant difference between them(P >0.05) .CEA mRNA positive rate was 50% (6/12) at a size <5cm, 62.5% (15/24) at a size 5=5cm; 0% (0/2) at localized type, 72.4% (21/29) at invasive type; 60% (6/10) at good differentiation, 57. 7% ( 15/26) at bad differentiation; 53. 3% (16/30) at invasive depth of m-ss, 83.3% (5/6) at invasive depth of se; 20% (1/5) at massive growth type, 64. 5% (20/31) at nest and diffuse growth type; the group of lymph node metastasis; 53. 3% (7/15) at N1, 73.7% (12/19) at N2; the number of lymph node metastasis; 45.5% (10/22) at ^6, 75% (9/12) at >6; TNM staging: 36.8% (7/19) at I + II stage, 80% (12/15) at IE + IV stage.
