Objective: By observing the effects of antiepileptic drugs (AEDs) on serum folic acid (FA) level and sister chromatid exchange (SCE) frequency in epileptic children, we study the mutagenesis effect of AEDs, and lood for effective way to prevent chromosome damage or mutagenesis induced by carbamazepine (CBZ) and valproic acid (VPA). Methods: There were totally 90 epileptic children were tested, which were divided into 4 groups: ⑴ CBZ group: 30 epileptic children treated with CBZ ; ⑵ VPA group: 30 subjects treated with VPA; ⑶ CBZ+N group: included 15 subjects administered with CBZ and nitrazepam; ⑷VPA+N group: included 15 subjects took VPA and nitrazepam; ⑸ Supplement FA group: included 20 subjects who were chose from CBZ and VPA groups at random. There was also a normal control group which have 25 healthy children. There's no significant difference in age and sex composition among each group. None of them had exposed to X-rays or had virus infections in the past month as well as liver, kidney or blood system disease history. By incubating peripheral blood lymphocyte, we observed the SCE frequencies, and atthe same time we tested the serum FA levels by chemiluminescent immunoassay. The two indexes of subjects in group ⑴ and ⑵ were compared before and after taking CBZ and VPA 3 month, 6 month, 12 month respectively, and that of allied groups were tested at 3 month, 9 month. Furthermore, we tested the two indexes after some of the subjects taking supplement FA (3 months generally). Results: 1. CBZ group: The results of SCE frequencies and FA levels according to different time points are as follows: when tested before administration, SCE frequency was 2.50±0.80/cell, FA level was 6.94±1.67ng/dl; after administration 3 month, SCE frequency was 6.90±2.17/cell ,FA level was 3.03±1.35ng/dl; 6 month later, SCE frequency was 7.44±1.70/cell ,FA level was 2.61±0.68ng/dl ; 12 month later, SCE frequency was 7.95±2.05/cell ,FA level was 2.46±0.98ng/dl. The average levels of the two indexes after administration: SCE frequency was 7.45±1.97/cel, FA level was 2.69±1.04ng/dl. The two indexes of control group: SCE frequency was 2.15±0.65 /cell, FA level was 7.23±2.98 ng/dl. The two indexes at each time point and the average level of them were significantly different from that of control group and before administration (P<0.05). There was'nt remarkable difference when compared between that of before administration and control group (P>0.05). While administered with CBZ, the SCE frequencies of the epileptic children raised, and theserum FA level declined as time went on. 2. VPA group: When tested before administration, SCE frequency was 2.53±0.85/cell, FA level was 6.68±1.78ng/dl; after administration 3 month, SCE frequency was 5.85±1.39/cell, FA level was 3.33±0.67ng/dl; 6 month later, SCE frequency was 6.44±1.45/cell, FA level was 3.05±0.59ng/dl; 12 month later, SCE frequency was 7.95±2.15/cell, FA level was 2.54±0.44ng/dl. The average levels of the two indexes after administration: SCE frequency was 6.80±1.91/cell, FA level is 2.94±0.75ng/dl. The two indexes at each time point and the average level of them were significantly different from that of control group and before administration (P<0.05). There wasn't remarkable difference when it was compared between that of before administration and control group (P>0.05). While treated with VPA, the SCE frequencies of the epileptic children raised, and the serum FA level declined as time went on.3. There is not remarkable difference between control group and the two groups before administrated CBZ and VPA (P>0.05).4. The average levels of the two indexes have negligible difference between CBZ group and VPA group after treatment (P>0.05).5. CBZ+N group: When tested before administered nitrazepam, SCE frequency was 6.25±1.41 /cell, FA level was 3.48±1.09 ng/dl.
