Objective To study the significance of nitric oxide (NO) and nitric oxide synthase (NOS) among diabetic nephropathy (DN) in patients with type 2 diabetes mellitus (DM).Methods NO and NOS of blood serum were determined in 122 cases of type 2 diabetes mellitus and 43 cases of controls. NO was assayed by the technique of nitrate reductase, NOS was measured by the spectrophotography, and URAC was determined by the technique of radio-immunity.Results NOS level of microalbuminuria group was significantly higher than that in control group and normalbuminuria group(P<0.01), and NO was significantly increased in microalbuminuria group as compared with that of three other groups(P<0.01). And it was found there were lower NO and higher NOS level in macroalbuminuria group than those in the other three groups(P<0.01). But there was no significant difference between normalbuminuria group and control group in NO and NOS.Conclusion (1) NO and NOS take part in the occurrence anddevelopment at early period of DN in type 2 diabetes mellitus patients. (2) At later period of DN, NO was decreased while NOS was increased, the reason for that perhaps is not the decrease of NOS, but the decrease of NOS's activity, or the destruction of NO in DN patients at later period.
