Objective: The complications caused by xenogenous transfusion become more and more in recent years.Among these complications,Hepatitis and AIDS derived from the infection of virus have become a serious social problem.In new century,our country will face the more severe situation such as defict of blood source and more serious infection after transfusion.So how to save blood is a important medical studying topic. Acute hypervolemic hemodilution(AHH) is a new way of saving blood that was put forth in the nineties of twentieth century, to compare with Acute normovolemic hemodilution (ANH),AHH can save time and cost,and reduce contamination of blood,it will be more widely spread to use in clinic gradually.But the systemic elementary investigation about it is very little in civil,the influence of AHH on hemodynamics, Constitution Oxygenation, interal homeostasis and important organs wasn't still be clarified in detail,and it's reliability and effectiveness of saving blood also has to be more dicussed.6%Hydroxyethyl starch(200/0.5) is a new generation of artificial colloid, and posses the unique effect of blocking for leakage of blood capillary except that it can obviously expand blood volume and ameliorate microcirculation filling and oxygen supply,and it's incidence of harmful response is very low within clinical dosage, so it is a better plasmatic substitute that was used in preoperative AHH and volume therapy at present.This topic is to aim directly at some questions of influence of AHH on physisological function. Two group of dogs were adopted middle degree AHH with 6%HES and lactated Ringer' solution(LR) ,so as to observe the influence of AHH with the 6%HES and LR on blood volume, hemodynamics,constitution Oxygenation and blood coagulation , and compare with the control group(AHH wasn't practiced on it) toobserve whether AHH can reinforce the body's tolerance for blood loss and reduce the xenogenous transfusion.The purpose of this study is to require into the reliability and effectiveness of preoperative AHH,in order to provide theoretical bases for it can be reasonablely applicated in clinic.Method: 18 dogs divided into three (A,B and C) groups randomly,with 6 in each group.In A group with 20ml.Kg-1 HES was infused intravenously at a rate of 25 ml. min-1 after induction( in A group,the blood volume increased by 25% and HCT decreased to 30%).In B group with the isodose of LR was infused at same rate.In C group,any solutions wasn't infused.The anesthesia induction and drug of anesthesia maintenance is all the same. 30min after induction,all the dogs were released blood by arteria with the rate of 15 ml.min-1 in three times of 10ml.Kg-1 each time.Cardiac Output(CO),Mean Arterial Pressure (MAP),Central Venous Pressure(CVP),and Surrounding Vascular Resistence(SVR) were monitored continually during the whole experient-Collect anticoagulative sample from arteriovenous after anesthesia induction and before AHH(Ti),30min after AHH(T2), SOmin after 10ml.Kg-1 blood was released(T3), 30min after 20ml.Kg-1 blood was released(T4), 30min(T5) and 90min(T6) after 30ml.Kg-1 blood was released for determining the following data: (D arteriovenous blood gas analysis,record the Haematocrit(HCT) and caculate the Oxygen Supply(DO2) and Oxygen Consumption(VO2). (D venous plasma Prothrombin Time(PT)and Activated Partial Thromboplastin Time(APTT). ゛rterial plasma Lactic Acid Concentration (LAC).The data were expressed as mean盨D.Statistical package (SPSS 10.0)was used for processing data J><0.05 was considered that the difference has statistical significant.Result:1. General dataThere were no significance difference among three groups on age,weight, baseline value of HCT,experiment time and volume of blood loss.2. Changes on hemodynamicsThere were no significant differences among three groups on MAP,CO,SVR and CVP at TI (P>0.05) .At T2,the MAP of Agroup reach to peak,but there was nosignificant differences with other two groups .At T3 to T&, the MAP of A group was higher than that of B and C groups (P<0.05) .
