The incidence of soft tissue injury of skin is high both at peacetime and wartime. Some tissues are so seriously damaged, contaminated and involved with multiple tissues that they are difficult to heal, which is a heavy load on patients, their families and society. Therefore, it is urgent to find some convenient and effective method to improve the healing rate and quality. Vacuum-assisted Closure (V.A.C.) came into being to meet the requirement. Since it was put forward in early 1990s,many papers have been published about its clinical application and experimental research at home and abroad. Our department has made a profound study of the mechanism of V.A.C. It has been proved that V.A.C can increase local blood flow, reparative cell proliferation and granulation, regulate matrix metalloproteinase and hyaluronic acid level, relieve edema, decrease bacteria and ameliorate circulation in human chronic wound. This experiment aims at making a further study of the effect of V.A.C on wound healing start, reparative cell proliferation and migration.An acute skin wound model of pig was used to examine the effects of V.A.C on ptoto-oncogenes , plasminoge activator(PA)cascade and growth factors of margin tissues by immunohisto chemistry. From six patients (2 cases paraplegic pressure sore, 2 cases venous ulcer, 1 case skin defect after trauma, 1 case non- healing wound after operation of breast cancer) hospitalized in our department with chronic wound treated with V.A.C, we also observed the effects of V.A.C on ptoto-oncogenes, plasminoge activator(PA) cascade and growth factors of margin tissues by ELISA , Western blot and immunohistochemistry.We studied as follows in three parts:1. This experiment aims at making a study of the effect of V.A.C on the expression of c-myc , c-jun and Bcl-2 protein in margin tissues of pigs with acute wounds and patients with chronic wounds. Result: We found that in general, the expression of c-jun , c-myc and Bcl-2 protein in margin tissues of pigs with acute wounds increased and peaked at day 1, 3, 9 respectively after treatment with V.A.C,then the expression of them presented rapidly downtrend,but the expression and staining of experiment group were obviously higher than that of control group. In margin tissues from 6 patients with chronic wounds ,the number of c-jun positive cells was more than that of c-jun positive cells after treatment with V.A.C,but the number of c-myc and Bcl-2 positive cells was less than that of c-myc and Bcl-2 positive cells after treatment with V.A.C.The number of c-jun positive cells became more with the prolonging of treatment with V.A.C, on the contrary, the number of c-myc and Bcl-2 positive cells became less.2. This experiment aims at making a study of the effect of V.A.C on the expression of uPA, uPAR protein in margin tissues of pigs with acute wounds and patients with chronic wounds.Result: We found that in general, the expression of uPA, uPAR protein in margin tissues of pigs with acute wounds increased and both peaked at day 3 after treatment with V.A.C.then the expression of them presented rapidlydowntrend,but the expression and staining of experiment group were obviously higher than that of control group. In margin tissues from 6 patients with chronic wounds ,the number of uPA, uPAR positive cells was more than that of uPA, uPAR positive cells after treatment with V.A.C,the number of uPA, uPAR positive cells became less with the prolonging of treatment with V.A.C.3. This experiment aims at making a study of the effect of V.A.C on the expression of PDGF , TGF-P1 protein in margin tissues of pigs with acute wounds and patients with chronic wounds.Result: We found that in general, the expression of PDGF, TGF-J31 protein in margin tissues of pigs with acute wounds increased and peaked at day 3,6 respectively after treatment with V.A.C,then the expression of them presented rapidly downtrend,but the expression and staining of experiment group were obviously higher than that of control group.
